SUBJECT: NRC INSPECTION REPORT NO. 50-498/93-02, 50-499/93-03

This refers to the special inspection conducted by Messrs J. I. Tapia and

R. J. Evans, of this of* ice, January 12-29, 1993. The inspection included a

review of activities authorized for your South Texas Project, Units 1 and 2,

facility. At the conclusion of the inspection, the findings were discussed on

February 1, 1993, with those members of your staff identified in the enclosed

report. Areas examined during the inspection are identified-in the report.

Within these areas, the inspection consisted of selective examinations of

procedures an'd representative records, interviews with personnel, and

observations of activities in progress.

Based on the results of this inspectic1, one apparent violation was identified

and is being considered for escalated enforcement action in accordance with

the " General Statement of Policy and Procedure.for NRC Enforcement Actions"

(Enforcement Policy), 10 CFR 2, Appendix C. Accordingly, no Notice of

Violation is presently being issued for this inspection finding.

The apparent violation involved several examples.of a failure to follow

procedures for performing self-verification. The first example is documented

in the Details section of the report. The second example was previously

considered an unresolved item in NRC Inspection Report 50-498/92-29;

50-499/92-29. The remaining six examples were identified under one unresolved

item in NRC Inspection Report 50-498/92-32; 50-499/92-32. This issue is of

concern because it represents the return of a negative trend in personnel

performance. Personnel are not strictly adhering to self-verification

requirements and, as a result, work has been performed on the wrong component,

wrong train, and, in one instance, on the wrong unit.

In addition to the apparent violation, an unresolved item was identified.

This unresolved item involved the failure to satisfy Technical Specification

requirements relative to Emergency-Diesel Generator (EDG) availability and

mode change restrictions. This problem resulted from an apparent lack of

adequate oversight in the performance of an activity that has previously'been

known to cause problems. This issue will remain unresolved pending further-

NRC review.

An enforcement conference to discuss this apparent violation has been

scheduled for March 8, 1993, in the Region IV office. The purpo< i of this

conference are_to discuss the example of the apparent violation, its causes

and safety significance, to provide you the opportunity to point out any

errors in our inspection report, to provide an opportunity to present your

proposed corrective actions, and to discuss any other information that will

help us determine the appropriate enforcement action in accordance with the

Enforcement Policy. In particular, we expect you to address those actions

that you have taken or plan to take to ensure that personnel cease-performing

work on wrong components. You will be advised by separate correspondence of

the results of our deliberations on these matters. No response regarding the

apparent violation is required at this time.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and its enclosures will be placed in the NRC Public Document Room.

The response directed by this letter and enclosed Notice are not subject- to

the clearance pro.edures of the Office of Management and Budget, as required

by the Paperwork Reduction Act of 1980, PL 96-511.

to discuss them with you.
