. Licenses of Subpart C of 10 CER Part 71 or an Agreenent State licensee subject to the rquirments of 10 CER Part 71, as rquiral bf 10 CER Part 150.20, a quality assurance progran approved bf the Co, mission as satisfyirg the prwisions of Subpart H of 10 CER Part 71 is required.
You shmald rquest renewal of your Quality Assurance (OA) progran at least 30 days prior to the expiration date. 'Ihis willt rwide for contiruation of your OAI rogran to satisfy part of the pewisions of Stibpart C of 10 CER Part 71 until a final deternination has been taken on your application.
Unless an exemption is prwided by 10 CIR Part 170.11, an application fee of $150.00, as rquired by 10 CER Part 170.31, should be included with your request for renewal.
