APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458 River Bend Station Permit: CPPR-145 Baseo on the results of an NRC inspection conducted during the period of

, . April 15-19 and April 29 - May 3, 1985, and in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), 49 FR 8583, dated March 8, 1984, the following violation was identified:

A; High Strength Steel Bolts Preload Criterion V of Appendix B to 10 CFR Part 50 states, in part, " Activities affecting quality shall be . . . accomplished in accordance with . . .

instructions, procedures, or drawings. . . ." This requirement is implemented by paragraph 3.2 in QAP-5 of the Quality Assurance Program Manual which asserts the same.

The specification, " Structural Joints Using ASTM A325 or A490 Bolts,"

dated May 8,1974, requires fasteners to have a preload.

' Contrary to the obove, an inspection of the " Job Inspection Torque" (JIT) which was used to verify preload found that, of 200 bolts in 72 different connections, inspected on 14 inspection reports (irs), there were 8 with ,

significant failures. A significant failure was defined as a nut slipping at less than 90% of JIT. The 8 failures averaged about 67% and ranged from 27% te about 88% of JIT.

This is a Severicy Level IV Violation. (SupplementII.D) (458/8540-01)

B. . Inspection Records Criterion XVII of Appendix B to 10 CFR Part 50 states, in part,

" Sufficient recorde chall be maintained to furnish evidence of activities affecting quality. . . ."

Contrary to the above, suf'ficient records were not maintained to furnish evidence of activities affecting quality in certain cases. In the seven irs, which were generated by six different inspectors, it was found that 29 of the 61 connections checked did not have satisfactory documentation.

Examples of structural inspection activities with this problem are:

1. The number of bolts found in a connection and also detailed on drawings did not agree with the number of bolts reported on the irs

r Notice of Violation 2. The description of the connection on the IR did not agree with the installed condition of the connection in 9 of the 61 connections checked. In general, the IR stated a member connected to an embedment when in fact the member did not terminate at an embedment.

3. The markup drawing did not agree with the IR in 8 of the 61 connections checked.

This is a Severity Level IV Violation. (Supplement II.D) (458/8540-02)

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
