APPENDIX A

NOTICE OF VIOLATION

' Gulf States-Utilities. -Docket: '50-458/85-23

River Bend Station Permit: CPPR-145

Based on the results of an NRC inspection conducted during the period of

March I through. April 30, 1985, and-in accordance with the NRC Enforcement

Policy (10 CFR Part 2, Appendix C), 49 FR 8587, dated March 1984, the following

violations were identified:

1.. Failure to Monitor Temperature

10 CFR Part 50, Appendix B, Criterion V, and the licensee's approved

quality assurance program require that activities affecting the quality of

safety-related systems and components be prescribed by and accomplished in

accordance with-approved instructions, procedures, and drawings.

Standard ANSI N510-1980, requires the .use of a _ temperature indicating'

'

device accurate and readable to + or - 0.5 F. to verify isothermal .

conditions for the direct measurement method of air cleaning systems duct

' leakage' testing.

' Contrary to the above, the procedure,used for the conduct of the HVAC

systems duct leakage testing at River Bend did not. require the use of a

temperature indicating _ device and no temperature monitoring was: conducted

-during leak testing performance.

This is a Severity Level IV Violation. '(Supplement II D) (458-8523-03)-

2. Failure to Calibrate Gas Meter in Range of Use

10 CFR Part 50, Appendix B, Criterion XII, and the licensee's' approved

quality assurance program require that measuring and testing devices used

in_ activities affecting quality be properly calibrated and adjusted at

specified periods to maintain accuracy within necessary limits.

Contrary to the above, the Singer Company Model DMT-325, Control

No.12789, gas meter used for HVAC duct air leakage. testing was not

calibrated in range of use (e.g., calibration performed at 10 cubic feet

and instrument used at 20 cubic feet per minute). Also, the vendor manual

for' the gas meter only gives a nominal capacity of approximately 5 cubic

feet per minute.

~3.- Failure to Follow Procedure

10 CFR.Part 50, Appendix B, Criterion V, and the licensee's approved

quality assurance program require that activities affecting the quality of

safety-related systems and components be prescribed by and accomplished in

accordance with approved instructions, procedures, and drawings.

Test Instruction (TI) 8' requires the supervisor of startup test to stamp

preoperational test procedures to establish notification points for

' operations quality assurance prior to approval of the test for

~

performance.

Contrary to the above, preoperational test procedure 1-PT-210, " Integrated

ECCS and Loss of Offsite Power," was approved for performance without the

~ procedure being stamped to establish notification points for operations

quality assurance.

This is a Severity Level V Violation. (Supplement II D) (458-8523-02)

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby

required to submit to this office, within 30 days of the date of this Notice, a

written statement or explanation in reply, includin (1) the corrective steps

which have been taken and the results achieved; (2)g: corrective steps which will

be taken to avoid further violations; and (3) the date when' full compliance
