INSIGHTS FROM SELECTED COMMON-CAUSE FAILURE EVENTS Enclosed for your information and use is a supplemental report on insights from potential and actual common-cause failures obtained from licensee event reports (LERs). This report is a supplement to AE00/E92-02 which we published in June 1992. It includes an additional 18 common-cause failure events from 1990 LERs and a reformulation of the summary tables that indicate potential corrective actions for each of the events reviewed.
The dominant corrective actions are similar to those discussed earlier, namely, substituting equipment having a larger design margin had a potential impact on about 56 percent of the events; performing comprehensive systems tests, insuring adequate train separation, and the use of diverse equipment each had a potential impact on about 27 percent of the cases; and the use of staggered surveillance testing impacted about 20 percent of the events.
Enclosed is a revision to the draft information notice (IN) that Vern Hodge developed in July 1992. Other activities delayed our response to the draft IN, but I believe the use of the supplemental report in the IN focuses the message in a more succinct fashion.
