Based on the information submitted to our office on July 10,1996, from the License Fee and Debt Collection Branch, we have made the appropriate name change to registration certificate (NR-104-D-101-S).
Please be advised that you must manufacture and distribute the product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate.
