SUBJECT: NRC INTEGRATED INSPECTION REPORT N0. 50-440/96-05 AND
Dear Mr. Myers:
On July 26, 1996, the NRC completed a routine safety inspection at your Perry
Unit I reactor facility. The enclosed report presents the results of that
inspection.
During the 7-week inspection period, one plant transient, involving the trip
of a reactor feedwater pump, occurred that impacted on the plant's operation.
The operations staff responded well to this event. However, we observed that
there is a need to address improvement opportunities with the radioactive
material control program, particularly with respect to ensuring that the
general workforce has a clear understanding of the program's requirements.
We are also concerned about a violation of NRC requirements that was
identified.
In this case, the inspectors identified that maintenance workers
and supervisors failed to promptly and formally report a deficiency in a
safety-related valve resulting in a delay in the corrective action process.
This violation is cited in the enclosed Notice of Violation, and the
circumstances surrounding the violation are described in detail in the
enclosed report. Please note that you are required to respond to this le'1. -
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
Based on the results of the close-out inspection of Generic Letter (GL) 89-10,
we have determined that your motor-operated valve (MOV) program and
implementation at Perry are acceptable, and meet the inter,t of Generic Letter 89-10.
Program documentation, test data, and margin improvement plans
provided an adequate basis to conclude that all GL 89-10 program MOVs would
perform their intended safety functions under design-basis conditions.
Accordingly, we are closing the NRC review of your program.
