In response to your letter dated January 25, 1993, enclosed is Quality Assurance Program Approval for Radioactive Material Packages No. 0109, Ravision No. 4. Your approval has been amended to authorize "use only" of transportation packagings, effective on the date of issuance.

Please note the conditions in the approval including that design and fabrication activities are not authorized by this approval.

We have also registered you as a user of Certificate of Compliance Nos. 5957, 6058, and 9098 under the general license provisions of 10 CFR S 71.12.

I have forwarded a copy of your letter to the Licensee fee and Debt Collection Branch for consideration of your annual fees.
