The purpose of this letter is u acument our conversations with members of yor Staff which occurred on Wednesday, January 27, 1993. These conversations concerned changes to the planned completion date for a fire protection modification effort (DC-3197) in progress at the station. DC-3197 addresses identified design / installation deficiencies associated with penetration seals, fire barriers, and fire dampers.

The deficiencies had been reported in LER 88-030.

In revision 3 of LER 88-030 dated September 3,1991, Waterford 3 stated that Design Change DC 3197 was approved for construction on September 24, 1990. The LER further stated that all phases (3) of DC-3197 were planned to be completed by January 31, 1993. Phase I of the modification consists of penetration seal and fire barrier rework.

Phase II consists of penetration seal rework and new fire damper installations.

Phase III constitutes engineering evaluations of those seals for which a tested configuration could not be provided during Phase II. Due to factors discussed below, none of the three phases of the modification were completed by January 31, 1993.

The remaining scope of work involves 10 out of 228 fire seals,15 out of 25 fire dampers and the installation of a new fire door.

The adjusted planned completion date is July 30, 1993.

Compensatory actions (fire watches) for impaired fire rated assemblies will continue until the discrepancies are corrected.

The primary factor contributing to the need for the six month extension of the planned completion date involves a site event documented in LER 90-019.

On December 12, 1990, with the plant operating at 100% power, it was discovered that work associated with DC-3197 had breached the Control Room Envelope, leaving the Control Room Ventilation system in an indeterminate state.

The system was declared inoperable. The breach occurred on December 5,1990, when a penetration fire seal was removed from ventilation fire damper FD-45 in accordance with the approved Design Change. A temporary seal was 9302120009 930209 3

This event resulted in a suspension of work on the modification (0C-3197) until a root cause and course of action could be determined.

Field work was delayed approximately 11 months while the design package was revamped to address the pressure boundaries and resolve significant comments made during review cycles. Following revision 1 to the design change package, field work was again started [end of November 1991) and progressed steadily.

Subsequently Revision 2 of the package was processed which encompassed Phase II of the modification (damper modifications).

The formal review of Revision 2 resulted in significant comments which involved additional evaluations, HVAC calculations, and reviews. The design phase was elayed approximately three months. Revision 2 of the modification (Phase II) d was approved by the Plant Operations Review Committee (PORC) on June 25, 1992.

By this time, the plant was preparing for the Refuel 5 outage and efforts were concentrated on the outage-dependent scope of the modification (10 fire damper replacements and three seals). Phase III of the modification (nc field work involved) was apprcved by PORC on February 4,1993.
