MILLSTONE COMBINED INSPECTION 50-245/92-29; 50-336/92-31;

50-423/92-28

This refers to the safety inspection conducted by Mr. P. Swetland and others of this office on

October 28,1992, through December 22,1992, at Millstone Station in Waterford, Connecticut.

The preliminary findings were discussed with Mr. S. S:: ace, the Unit Directors, and others of

your staff at the conclusion of the inspection. Areas examined during the inspection are

described in the enclosed report. Within these areas, the inspection focused on issues important

to public health and safety, and consisted of performance observations of ongoing activities,

independent verification of safety system status and design configuration, interviews with

personnel, and review of quality records.

During this inspection, we noted good performance in the implementation of design reviews and

'

testing of the Unit 2 heating, cooling, and ventilation systems, and in the preparation and conduct

of the Unit 2 refueling operations and steam generator post-weld heat treatment.

Your identification and proposed correction of the failure to conduct timely Unit I reactor vessel

surveillance capsule analyses were evaluated during this inspection. Likewise, your response to

the Unit I reactor trip caused by an operator failure to follow procedures during a surveillance

test was reviewed. We determined that your corrective actions were acceptable, and the events

had minor safety significance. Therefore,in accordance with Section VII.B of the Enforcement

Policy, enforcement discretion was exercised for these cases and no violations were cited.

During this' inspection period, the inspectors identified that certain requirements of your

administrative control procedures were not adequately implemented. These included: (1)

periodic review and reporting of temporary modifications installed for a prolonged period; (2)

safety evaluations for some scaffolding installations; and (3) revision of design documents

following completion of modifications such that required Final Safety Analysis Report updates

are submitted in a timely manner. In addition, the licensee failed to implement committed

corrective action following a' 1990 Unit 2 mispositioned valve event. As a result, another

inadvertent repositioning of a Unit 2 valve caused a momentary loss of shutdov a cooling during

this inspection. Three violations of NRC requirements concerning these problems are cited in

the enclosed Notice of Violation.

You are required to respond to the enclosed Notice of Violation and should follow the

instructions in the enclosed Notice when preparing your response. In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and the enclosed Notice will be

placed in the NRC Public Document Room. The responses directed therein are not subject to

the clearance procedures of the Office of Management and Budget as required by the Paperwork

Reduction Act of 1980, Public Law No. 96.511.
