SUBJECT: COMBINED INSPECTION REPORT NOS. 50-352/92-29 AND 50-353/92-29
This letter transmits the results of the resident safety inspection by Messrs. T. Kenny and
T. Eastick, from November 15,1992 - January 2,1993, at the Limerick Generating Station,
Limerick, Pennsylvania. The inspection focused on the observation of activities important to
ensuring the public health and safety. Reviews of documents and interviews of plant
personnel were also performed. The results have been discussed with Mr. J. Doering of
your staff.
The attached report discusses the reactor trip of December 4,1992 and the maintenance
efforts to return the unit to service. Your operations staff performed the shutdown in an
orderly manner with good command and control. During this period, there were a number
of management meetings. Some of the management meetings were requested by your staff
and yourself. We found the information informative and helpful in understanding your
present program for the Nuclear Department re-organization. We also appreciated the -
advanced plans on how you plan to deal with radiological concems during the upcoming
refueling outage.
You will note in Section 3.4 of this report the documentation of a failure to follow
procedures in the maintenance area. This is another example of our concerns in the area of
maintenance that were discussed during the management meeting of December 22,1993.
Since this event occurred before the above meeting, and Section VII.13.2 of the Enforcement
Policy was satisfied, this event has been classified as a non-cited violation.

I am also concerned about the significant number of times you routinely enter LCOs
(Limiting Conditions for Operations) for maintenance activities on safety-related equipment.
(Section 5.0) It'is our intent to perform an inspection of this aspect of your program in the
-
near future. A management meeting to discuss this matter may be useful following our
inspection.
