NRC INSPECTION REPORT NOS. 50-348/92-25 AND 50-364/92-25
This refers to the inspection conducted by G. F. Maxwell of this office on
October 5 through November 9, 1992.
The inspection included a review of
activities authorized for your Farley facility. At the conclusion of the
inspection, the findings were discussed with those members of your staff
identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The enclosed inspection report identifies activities that violate NRC
requirements that will not be subject to enforcement action because the
licensee's efforts in identifying the violations meet the criteria specified
in Section Vil B. of the Enforcement policy.
However, we are concerned about
these three events because they indicate a need for increased attention to
detail by the individuals involved. We recognize that your " STAR" (Stop,
Think, Act, Review) program has been effective in preventing additional
events; however, increased effort is need to minimize these type events and
"near misses".
Particular emphasis should be directed toward the individuals
and managers involved in these (and previous events) to assure that the root
cause(s) of these type problems are identified and corrected,
in accordance with Section 2.790 of tue NRC's " Rules of Practice," a copy of
this letter and its enclosures will be placed in the NRC Public Document Room.
