SUnJECT: INSPECTION REPORT NOS. 50-245/92-30,50-336/92-32, AND 50-423/92-29

This letter refers to the inspection of engineering and technical support activities conducted -

during the period November 16-20,1992, at Berlin and Waterford, Connecticut by

Mr. A. Lohmeier. The inspection related to the retention of records for those facility

components designed for a limited number cf transients or cycles at Northeast Nuclear

Energy Company Millstone Nuclear Power Station Units No.1,2, and S.

Mso included in

the inspection was the response of Northeast Nuclear Energy Company in providing the

required information of Generic Letter 92-01 relating to reactor vessel belt-line material-

adequacy expressed in 10 CFR 50 Appendices G and H. Mr. Imhmeier discussed the results

of the inspection with members of the Northeast Utilities Nuclear Energy Company

engineering and licensing staff at the inspection exit meeting on November 20,1992.

Further telephone discussion with the Northeast Nuclear Energy Company engineering staff

on December 18,1992, provided additional information and clarification related to-

operational cycle monitoring sufficient to complete the inspection on that date.

The monitoring of transients or cycles over the lifetime of components designed for a limited

number of cycles is important to the safety assessment of equipment during and beyond the

expected facility lifetime. Our inspection revealed both strengths and weaknesses in

Northeast Nuclear Energy Company programs to monitor cyclic operation. Inspection of

Millstone Unit 3 revealed a system in place for tracking the numbers of operational cycles

and transients. On Millstone Units 1 and 2, however, there was found no system to convert

records of transient or operational cycles to the fatigue life assessment of components

!

designed for a limited number of transient or cycles. While the operational data was

recorded in accordance with Technical Specificationn Section 6.9.1.6, the data had not been

applied toward assessment of fatigue life of components since 1982. Furthermore, on

,

Millstone Unit No.1, on the basis of calculations performed in 1982, estimates of present

fatigue life usage factor approach the allowable value of 1.0 for the reactor basin seal skirt.

The inspector found that Northeast Nuclear Energy Company engineering is considering

installation of a computerized system for transient cycle monitoring in 1993, that would

provide for monitoring and evaluation oi components designed for a limited number of

transients or cycles on Millstone Units 1 and 2.

In discussion of this matter with members -

of your engineering department on December 18,1992, it was requested that Northeast

,

Nuclear Energy Company engineering provide analytic justification that the design of the

reactor basin seal skirt of Millstone Unit I will not reach end of fatigue life during the

anticipated life of the facility. This issue will be considered as an unresolved item.
