Dear Mr. Mikowski:
This is in response to your letter dated August 24, 1984, in which you requested, pursuant to the Freedom of Infomation Act, copies of the radioactive materials license applications with all supporting documentation and license amendment applications for Nuclear Pharmacy Inc., Chicago, Illinois; Pharmatopes, Inc., Chicago, Illinois; and American Testing and Inspection, Mokena, Illinois.
We have been infomed by the staff that when you were at Region III you indicated to a member of the Licensing Staff that, in view of the extremely. broad nature of your request, you were willing to narrow the scope of the request to the application materials back to the last renewal of the licenses for Nuclear Phamacy, Inc., and for American Testing and Inspection, and to withdraw that portion of the request for the Pharmatopes, Inc., records.
Also, in a telephone conversation on October 1,1984, with Mrs. Pappas of igy staff, you agreed to deletions of personal privacy infomation, as indicated on Appendix B, and to pay for professional search and reproduction charges.
Enclosed are the documents listed on Appendices A and B which pertain to your request.
The professional search charge, as specified in 10 CFR 9.14(b)(3),
totals $72.00 (6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> of professional search at $12.00 per hour).
The charge for reproducing reco> ds not located in the NRC Public Document Room is seven cents ($0.07) per page, as specified in 10 CFR 9.14(b).
Accordingly, the cost of reproducing the 730 pages provided to you with this letter is $51.10. The search and reproduction charges total $123.10.
You will be billed for this amount by our Division of Accounting and Finance.
