This refers to the results of the routine resident safety inspection conducted by Messrs.

W. Cook, J. Tappert, L. Scholl and Ram Ilhatia from November 15,1992 to

December 19, 1992 at James A. PitzPatrick Nuclear Power Plant, Scriba, New York.- A- ,

summary of the inspection Dndings was presented to you and members of your staff at the .

conclusion of the inspection.

This inspection was directed toward areas important to public health and safety. Areas

examined during the inspection are described in the NRC Region I inspection report, which

is enclosed with this letter. The report period has been extended to December 24,1992, to

encompass additional NYPA and NRC activities discussed in the report pertaining to plant

restart. Within these areas, the inspection consisted of observation of activitics, interviews

with personnel, and document reviews.

As discussed in the enclosed report, NYPA's review of the relay room CARDOX test failure -

and reactor water cleanup valve repair determined that NYPA personnel performance .

deficiencies contributed to the problems encountered with these events. -You and your staff's

continued efforts to be more self critical are clearly evident in NYPA's followup of these - <

recent CVents.
