COMBINED INSPECTION 50-277/92-32 AND 50-278/92-32

This letter transmits the findings of the resident safety inspection conducted by Messrs. J. J.

Lyash and F. P. Bonnett, and Ms. M. G. Evans and Ms. B, E. Korona from December 15,

1992, through January 18,1993, at the Peach Bottom Atomic Power Station, Delta, Pennsylva-

nia. The inspectors based thcsc findings on observation of activities, interviews and document

reviews, and discussed them with Mr. K. P. Powers of your staff.

During the period, your staff identified that an emergency service water system breaker had

been positioned incorrectly following completion of maintenance activities. This error apparent-

ly resulted in a condition that was not consistent with your 10 CFR 50, Appendix R, safe shut-

down analysis. You promptly reported this event, and began an investigation that identified

several contributors. Our preliminary review identified additional weaknesses that warrant your

attention, as discussed in the attached report. We are aware that your internal event investi-

gation process has previously identified component mis-position events as a general area of

concern, and that management has focused attention on its resolution. We plan to perform

additional inspections of this recent event to evaluate its significance with respect to safe

shutdown capability, your assessment of the specific weaknesses identified by the NRC inspec-

tors, and the corrective actions implemented or planned in response to the broader component

,

mis-position issue that you identified. In accordance with 10 CFR 2.790 of the NRC's " Rules

of Practice," a copy of this letter and its enclosures will be placed in the NRC Public Document -

Room.
