) QN M MINUTES OF THE MEETING 0F THE ACRS SUBCOMMITTEE ON QUALITY AND QUALITY ASSURANCE IN DESIGN AND CONSTRUCTION JANUARY 3, 1985 A meeting was held by the ACRS Subcommittee on Quality and Quality Assurance in Design and Construction at 1717 H Street, Washington, D.C.,

Room 1167. The purposes of this meeting were 1) to review Regulatory Guide 1.28, Revision 3, " Quality Assurance Program Requirements (Design and Construction)," for final approval; 2) to discuss NRC/IE's Inte-grated Design Inspection Program and the Independent Design Verification Program; 3) to review the Quality Assurance research effort for input into the ACRS Report to the Congress on Reactor Safety Research; and 4) to discuss the QA program plan, a three-year plan for NRC QA/QC initiatives. (The discussion of the QA program plan was deferred.)

Notice of this meeting was published in the Federal Register on Friday, December 21, 1984 (Attachment A). The schedule of items covered at this meeting is in Attachment B. Attachment C is a list of the meeting hand-outs which are in the ACRS office files. Portions of this meeting that dealt with the proposed NRC research budget for FY 1986 were closed.

Such discussions if held in public session might result in premature disclosure of information which would, in turn, frustrate the Comis-sion's ability to implement the affected programs effectively.

Mr. Richard Major was the cognizant ACRS staff member present for the meeting.

The Subcommittee reviewed Reg. Guide 1.28, Revision 3, " Quality Assurance Program Requirements (Design and Construction)," for final approval. The Subcommittee will recommend this guide be approved. The Guide endorses ANSI /ASME NQA-1-1983 with supplementary positions.

The guide has three regulatory positions. Position 1 deals with qualification of inspection and test personnel. It provides supplementary guidance on education and experience requirements.

Position 2 concerns quality assurance records; it provides standardized retention times (listed in table one at the back of the guide) and definitions of records. Position 3 addresses audits. It specifies frequencies for internal and external audits.

QUALITY / QUALITY ASSURANCE 3 January 3, 1985 Meeting Discussion of Integrated Design Inspection Program and Independent Design Verification Program i

The Integrated Design Inspection (IDI) and Independent Design Verification Program (IDVP) were discussed. The IDI is a direct inspection by the NRC Staff. The Staff is able to perform about three IDIs per year. The IDVP is'a third party review that is independent of the applicant's other programs. As a result of the Diablo Canyon

" mirror image" mistake, the Staff is requiring this additional assurance that plant designs meet the FSAR and Applicant Commitments. The performance of an IDI or IDVP is determined on a case-by-case basis.

Although there is no legal requirement to perform an IDI or IDVP, such a program improves the ease of the licensing process.

IDIs are performed by a team of from 10-15 NRC inspectors with contractor assistance. The inspections address five disciplines:

mechanical systems, mechanical components, civil / structural, electric power, and instrumentation and controls. The inspections emphasize the implementation of FSAR commitments, technical adequacy of design documents, design interface control, and design change control. The Staff involvement includes inspection preparation. Direct inspections are conducted at applicant's offices, the construction site, 4/E Offices (where most of the time is spent), NSSS Offices, and subcontractor

. offices. The Staff prepares inspection reports and follow-up of the inspection report findings. The IDI process takes about four months.

Members of the IDI teams are well qualified with many years of nuclear power plant experience. The experience includes A/E and NSSS design ,

l The IDVP is essentially the same as the IDI except that an independent I contractor (a third party) is picked by the applicant to perform the  !

tt QUALITY / QUALITY ASSURANCE 4 January 3, 1985 Meeting program. The Staff reviews the selection of the independent contractor, the program review plan, plan implementation, the completed report, the applicant's response to review findings, and prepares a safety evaluation report.

The IE review of IDVP reports includes: an assessment of the depth of the independent review, a determination that the generic implications of findings have been properly addressed for systems outside the IDVP.

Quality Assurance Program Plan The' Subcommittee decided to defer discussion of the QA Program Plan until a later date.

Review of the Ouality Assurance Research Effort Due to the modest size of the QA Research effort, the Subcommittee decided not to include comments on this effort in the annual report to Congress on the Safety Research Program.

QA Effectiveness Research has been conducted to determine the costs and benefits associated with QA Programs and Activities. It is attempting to quantify the relationships between quality, quality assurance, and safety. (For several years the ACRS has been interested in ways to measure the benefit of QA activities.) The projects conducted to ,

evaluate QA effectiveness faced the following limitations: the effect of QA activities alone is hard to isolate, especially when QA is highly integrated with plant operating procedures and there are no generally accepted approaches for quantifying QA effectiveness, and there is

incompleteness and inconsistency of data sources. In the limited research results reported on, there was no statistically significant difference in performance between safety grade and nonsafety grade components.

Research in Systems Assurance Analyses (SAA) is designed to examine new techniques to improve the basis for the "Q-List," those items that must

. . meet 10 CFR 50 Appendix B requirements. SAA attempts to provide a structured basis for identifying systems and subsystems which have a safety'effect and which require the application of quality requirements.

This program is ongoing.

. Research is underway to ~use _PRA to establish quality program applicability. It examines expanding uses of existing PRAs to_ identify and provide relative ranking of safety equipment and operator actions.

This effort will be used to improve the "Q-List." All equipment is ranked according to its safety significance in preventing and mitigating internal events using PRA fault-trees and event-trees. A second pass considers special emergencies (external events). Results of the two passes are combined to provide relative, plant-specific rankings of equipment activities. This program is ongoing.

An effort-is being conducted to determine the scope and significance of the problem of unqualified QA/QC personnel performing quality functions

- in the nuclear industry. The work in Oualification/ Certification of QA/QC personnel is to determine the causes of the problems and recommend solutions commensurate with the significance of the problems identified.

A research effort is underway on the future nature of the r.uclear ,

industry. Its purpose is to determine the changes which are likely to occur in the nuclear industry during the period of 1985-1995. The quality implications of the changing utility-contractor-vendor-supplier infrastructure are the focus of the study. The goal is to project how
