Dear Mr. MacDonald:
By copy of the enclosed letter, we have notified the 11ew York State Department of Labor that Eastman Kodak Company wichen to terminate IJYS DOL Licence 11o. 1941-1929 for use of radioactive materials in chielded room industrial radiography operations at Kodak's Marketing and Education Center in Rochester, NY.
In annociation with this operation, we participated in the
!!RC Quality Ansurance Program for Radioactive Material Packages under Approval No. 0092.
This program is also being terminated.

31 Decenber 1992 George Kasyk New York State Department of Labor Division of Safety and Health Radiological Health Unit, Room 813 One Main Street Brooklyn, NY 11201 RE: NYS DOL Radioactivo Material License No. 1941-1929 Dear Mr. Kasyk As per your recent telephone conversation with Radiation Safety Officer John H. Heyer, please be advised that Eastman Kodak Company wishes to terminate the above-captioned license, effective immediately.
The sealed radioactive msterial sources held under this licenr,e were renoved from the Industrial Imaging Laboratory at our Marketing Education Center's Riverwood complex in December, 1991; copies of the receipts are attached.
Note that we will continue to conduct shielded room x-radiography at this location using radiographic equipment which is' registered with the Department of Labor and which meets the criteria outlined in 12 NYCRR 38.
