There is an NRC sponsored public meeting in Plymouth, Massachusetts schedu?ed l for February 3, 1993. The meeting was requested by the Selectmen from the towns of Duxbury and Kingsten to update the officials and local residents on the current status of the notching problems associated with the reactor vessel water level instrument expericnced at Pilgrim. There are two inspections at Pilgrim this week, one is an inspection of the Emergency Operating Procedures and the other is a pipe support inspection. Tro.ian Nuglear Plant Two meetings have been scheduled with Portland General Electric Company (PGE), ' following PGE's, January 4,1993,. decision to permanently cease operation of Trojan Nuclear Plant. The first meeting will be held at 1:00 p.m. on January 20, 1993, in Region V offices to discuss PGE's decommissioning-plans, staffing resources, and related licensing actions. The second meeting will be held on January 21, 1993,-in Portland, Oregon, with the Energy Facility Siting Council to allow PGE to discuss it's decommissioning plans with the State  : officials. Currently, PGE is planning to submit its Possession Only License to NRC, by January 26, 1993, and start removing fuel from the core during the first week l of February 1993. Oconee 1 Steam Generator Insp_tction Results (EMCB) In response to a recommendation from Babcock and Wilcox, Duke Power changed 4 l its past-practice at Oconee 1 for bobbin coil eddy current indications. In - i the past they had treated signal-to-noise ratio (S/N) of less than 5 as being l non-relevant. Low S/N signals are now subjected to motorized rotating pancake l coil (MRPC) followup inspection, and if confirmed by MRPC, the tube is l plugged. Duke has just completed 100% eddy current inspection of the tubes in - j their A and B steam ger.erators (SG), and identified-139 and 473 respectively, pluggable tubes. A reassessment of past inspection results revealed'that most of the indications were previously detected but were considered as being non-relevant. Depth measurements are highly uncertain for these low.S/N ratio . indications with some indicating through-wall; however, the indications are quite localized which may mitigate their tube integrity implications. Recent tube pulls at other B&W units have shown that low S/N ratio indications may be associated with crack depths greater than 40%. The staff is following this occurrence for potential generic implications. I l - JANUARY 15, 1993 ENCLOSURE A ,

2 IyA 1/7/93 Fire Test Re sits (SPLB) On January 7, 1993, at Omega Point Iabs, TVA performed standard ASTM E-119 l-hour fire endurance test and hose stream tests on 1" and 5" conduits protected by an enhanced Thermo-Lag Fire Barrier System. The licensee considers these preliminary test results as successful. During the test the highest internal temperature experienced was 265 degrees Fahrenheit (l" conduit). In addition, TVA plans to verify that the cables would have passed the fire endurance acceptance requirement of 325 degrees Fahrer.heit coldside temperature through compression load test. The success of the test specimen enhancements will be based on temperature profiles recorded and future compressive load tests. TVA plans to test a conduit assembly in March 1993 with one, two, three and four-inch conduits installed. The one and two inch specimens will have 3/4-inch thick Thermo-Lag material installed, the three and four inch conduit specimens will be tested with 1/2-inch Thermo-Lag material installed only (no enhancements). Westinahouse Miniflow Meetino - A meeting was held on January 7, 1993, with Westinghouse and several Westinghouse owners to discuss the use of relief valves-for mini-flow control in the charging system. This neeting was held as part of the staff's longer term generic follow-up in rerponse to operational problems with the mini-flow system in the Shearon Harris plant. Facilities represented at the meeting included Beaver Valley, Comanche Peak 1 and 2, Millstone 3 and Vogtle 1 and 2. The staff learned that all of these . plants use relief valves with rated capacity of about 96 gpm. This is much . smaller than the 270 gpm capacity of the Shearon Harris valves. In addition, l all except Beaver Valley (and Shearon Harris) have intermediate-head safety injection pumps, in addition to high pressure injection (charging) pumps. Because of the icw capacity of the relief valves, diversion of fl.ow through open relief valves in the mini-flow system will not prevent _ the high pressure portions of the emergency core cooling system from performing their intended function with a mini-flow valve failure. Plants which also have intermediate head pumps are more than adequately protected from valve failure arid also well protected from diversion via a ruptured drain line. Each licensee presented a history of performance, surveillance, and operability determinations for their mini-flow systems. All licensees concluded that the systems in their plants have been operable and continue to - be operable. All licensees argued that at the lower rate of mini-flow, i.e., 96 vs 270, the valve chatter and other significant effects experienced at Shearon Harris would not occur. The utilities referenced a Comanche Peak test (Fall,1992) to confirm this. This is a significant finding since valve chatter at Shearon Harris led to failure of the drain line that could have diverted hundreds of gallons of emergency core cooling system flow per minute. All licensees reported that they had made enhancements in the administrative treatment of the mini-flow system. However, only Beaver Valley proposed an

3 emergency operating procedure enhancement to address high pressure. injection (HPI) degradation due to a -failure or malfunction-in the mini-flow system. This is because only Beaver Valley relies solely on HPI-charging pumps without intermediate-HPI backup to perform the HPI function. In addition, all plants except Comanche Peak proposed enhanced ~ surveillance testing of the mini-flow relief valves, i.e., one valve each ' refueling outage versus both valves every 5 years. Based on the information presented at the meeting, the Reactor Systems Branch has concluded that no further regulatory action is necessary for plants other than Comanche Peak. The only remaining issue for Comanche Peak is inservice testing. The Mechanical Engineering Branch has the lead for this issue because it pertains to the inservice testing program. il JANUARY IS, 1993 ENCLOSURE A l

l Office of Nuclear Material Safety and Safeguards items of Interest Week Ending January 15, 1991 Meetina Renardina NUHOMS _MP187 Splai_{ge] TranseatLPEtne On January 12, 1993, the Transportation Branch staff met with Pacific Nuclear and Sacramento Municipal Utility District to discuss the NUHOMS MP187 spent fuel transport package. The NUHOMS-MP187 is a stainless steel and lead shleided cask which is being designed to transport NUHOMS dry shielded storage canisters. The cask will accommodate a single NU110HS canister, which holds 24 PWR fuel assemblies, and will be transported by rail. The discussion centered on the design and testing of the impact limiters. htilpa with.. Combustion Enaine;>rino Representatives of the International Safeguards Branch and Uranium fuel Section met with the manager of Nuclear Materials Licensing, Combustion Engineering to discuss the Design Information Questionnaire (DlQ) and criticality safety for their planned consolidated facility in Hematite, Missouri, which is scheduled to begin production in September 1993. The D10 is required to be submitted to the International Atomic Energy Agency (IAEA) because of the facility's selection under the Protocol to the US/lAEA Agreement. Presently, the Windsor, Connecticut facility fabricates fuel assemblies from uranium dioxide pellets that are supplied by the Hematite facility The new consolidated facility will encompass uranium hexafluorido conversii '1 rough fuel assembly f abrication. Three goai. established for the Windsor facility by the parent company are: (1) divestiture of nost of the nuclear material by the end of September 1993, but continuing laboratory operation with an inventory of less than 350 grams of U-235; (2) decommissioning and decontamination of the manuf cturing operations; and (3) reduction of the annual licensing fee. federal Ayhilqp Administration Memgaply3Lof Undentapjjng On January 7,1993, General Steele, Assistant Administrator of Civil Aviation Security, Federal Aviation Administration (FAA), signed the revised Memorandum of Understanding (MOV) between FAA and the Nuclear Regulatory Commission. 1he revised M00 clarifies the roles and procedures for notification and exchange of threat information between the NRC and the FAA. Il Insoectori The Divi ion of Safeguards and Transportation (SGTR) held an interface meeting with CAT 1 physical security management and Region 11 physical security inspectors on January 12, 1993. The meeting allowed attendees to discuss their experiences and/or concerns on tactical response training exercises, as well as other topics of general interest. s I 2 i Visit to low-level Radioactive Waste Storage facility at Calvert Cliffs On January 6,1993, staff of the Division of Low-Level Waste Management and Decommissioning visited the Calvert Cliffs Nuclear Power facility. The objective of the visit was to tour the Calvert Cliffs Materials Processing facility (MPF) and operations relative to low-level radioactive waste (LLW) management and storage. The MPT was constructed in 1985 and was designed in accordance with Generic letter 81-38, " Storage of Low-Level Radioactive Wastes at Power Reactor ' Sites", to provide interim storage of dry active waste for p to five years. The MPF has LLW storage capacity of approximately 75,000 ct.oic feet. Calvert Cliffs currently ships most of its generated waste (approximately 36,000 cubic feet per year) to waste processing f acilities that reduce the volume of waste to approximately 15 percent of the total generated at the site. Pter corrpaction and incineration by the waste processors, dry active waste approximating 4,500 cubic feet per year is sent -to the Barnwell Low-Level - Radioactive Waste Disposal Facility for disposal. De-watered wastes, which comprise approximately 1,500 cubic feet per year, are sent directly from Calvert Cliffs to Barnwell for diiposal. Th' '4PF also contains processing equipment capable of sorting, segregating, and compacting dry active txite. Facility management, however, currently uses outside waste processors to treat generated LLW prior to disposal at Barnwell. Calvert Cliffs foresees greater reliance on storage of LLW at the MPF beyond June 30, 1994, the date by whan the Barnwell disposal facility is expected to close to states located outside the Southeast Compact. Maryland is a member of the Appalachian State Compact (Pennsylvania is the host State) and the compact's LLW disposal site is scheduled for operation in 1997.  ; 3 Dry Scent fuel Cask Rulemakina , Pacific Sierra Nuclear and Transnuclear, two vendors in the subject rulemaking, have been informed of the Nuclear Regulatory Commission's proposed determination not to withhold from aublic disclosure certain proprietary information st.bmitted in the rulema'.!ng. The proprietary information would , subsequently form the basis for the final rule to add two c'tsks to' the " List of Approved Spent fuel Storage Casks," (10 CFR 72.214) eligible for use.under the general license in Subpart K of 10 CFR Part 72. This propesed determination was made pursuant to Section 2.790(c) of NRC regulations which provides that "information submitted in a rulemaking proceeding which subsequently forms the ba:.is for the final rule will not be withheld from public disclosure by the Commissicn and will not be returned to the applicant after denial of any application for withholding submitted in connection with--

Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending January 15, 1,93 Q11qngstic Evalqg' W Incideallayntiaation Etanch (QUlEl The incident investigation Team (111) investigating the medical misadministration and loss of iridium-192 source from an Omnitron 2000 high dose rate brachytherapy afterloader is continuing work on its report. The 11T's medical consultant submitted his report this week. Southwest Research Institute, an NRC contractor supporting the IIT, submitted an interim report on their studies on the cause of the Omnitron source wire failure. Their final determination of cause of the failure will nt,t be completed before issuance of the llT report on January 29, 1993. Omnitron also submitted a separate report on their studies of the wire failure. Cytrogentic dose results have been received for 8 of 9 exposed individuals. Results to date are below the threshold for a confirmed positive for any one individual which indicates no one received a dose significantly above 20 rems. J Eident i Respance Branch flRBI During the weeks of January 4-8 and 11-15, IRE staff and Halliburton-NUS Eny,ronmental Corporation personnel did final computer testing for the Emergency Response Data System (CRDS) for 16 reactor units: San Onofra 2 and 3, Summer, Brunswick 1 and 2, Shearon Harris, Browns ferry 2, Sequoyah I and 2, Haddem Neck, Milestone 1, 2 and 3, Salem 1 and 2 and Hope Creek. At this time 48 sites comprising 71 reactor units are connected to the ERDS. On January 12-13, COMEX, an NRC contractor, presented 2-day training classes on Response Technical Manual-92 (RTM-92) to members of the Protective Measures and Reactor Safety teams at NRC Headquarters. Representatives from the Pennsylvania Power and Light Company and from other federal agencies also attended. On January 14, the 00A Division Director and an IRB staff person attended the Catastrophic Disaster Response Group (CDRG), chaired by the federal Emergency Management Agency (FEMA) and comprised of representatives from the various federal agencies responsible for incident response in the U.S. The purpose of the meeting, which was held in Washington, D.C., was to share lessons learned from the emergency management experiences of Hurricanes Andrew and iniki and Typhoon Omar. AE0D emphasized the need for completing the ongoing efforts to proceduralize the integrated federal response activities associated with nuclear facilities, Additionally, this maeting provided en opportunity for FEMA to recognize those individuals throughout the federal comunity who contributed to the development and establishment of the Federal Response plan (FRP). Two members of the NRC, Mr. Lee Spessard and Ms. Rosemary Hogan, along with individuals from the other signatories to the FRP, were honored with a plaque and the personal recognition of Messrs, Grant Peterson and Wallace E. Irgnd] and Patterns Analysis Branch (TPAB1 On January 8,1993, Trends and Patterns Analysis Br6nch (TPAB) staff briefed the ACRS on SECY-92-425, " Performance Indicator Program - Peer Group and Operating Cycle Phase Enhancements." The briefing covered the background, development, and trial evaluation of the changes proposed by AE0D to incorporate peer groups and operating cycle phase effects into the P1 program. A meeting was held on January 14, 1993 at the Office of Management and Budget with members of the Contract Administration Staff and representatives from other agencies which sponsor FFRDCs. The aurpose of the meeting was to discuss the recommendations contained in tie Congressional report entitled,

              " Inadequate Federal Oversight of Federally Funded Research and Development

, Centers." The report's recommendations are aimed at strengthening Federal controls over FFRDCs. lwo White Flint North Meetino I The Division of Contracts and Property Management and the Office of Consolidation staff met with GSA representatives on January 12, 1993 regarding  : the role of GSA in conjunction with the move to Two White Flint North. Discussion centered on services which are offered by GSA, as well as, funding responsibilities. Procedures for removal and disposition of property from ' vacated buildings were also discussed. , t Self-Guarantee as an Additional Financial Assuritnce Mechanism A proposed rule entitled, "Self-Cuarantee as an Additional Financial Assurance Mechanism" was published in the Federal Register on January 11, 1993 (56 FR 3515). The proposed rule would amend regulations-for decommissioning licensed facilities to allow certain non-electric utility licensees to use self-guarantee as a means of financial assurance. The proposed rule is intended to reduce the cost burden of financial assurance while continuing to provide the r NRC with sufficient assurance that decommissioning costs will be funded. The ! comment period for the proposed rule closes March 29, 1993. {onduct of Emoloyees: Conformina Amendments A final rule entitled, " Conduct of Employees; Conforming Amendments" was published in the Federal Register on January 12, 1993 (58 FR 3825). The final rule removes provisions from the NRC's regulations governing the conduct of its employees that have been superseded l'y regulations recently issued by the Office of Government Ethics. The final rule becomes effective on February 3 ' 1993. L NRC's Druo Testina Prooram NRC continues to randomly test employees in testing designated positions (TDP) at a frequency of 10 times per year and at an annual rate of 50% (number of L annual tests is equivalent to one half NRC's TDPs). NRC employees at nuclear i power plant sites are randomly selected by site. Since employees in TDPs and L sites have ben equal chance of being selected each of the ten times, some i employees and sites are selected more than once per year. It will be applicable , to all software developed for in-house use by the NRC or for the NRC by contractors. The successful completion of this project, lasting almost four years, is a significant milestone in NRC Software Quality Assurance activities. [ross-Servicina for Payroll / personnel SerVicel NRC has selected the Department of Health and Human Services (HHS) to provide cross-servicing for payroll / Personnel processing using their

                " IMPACT" integrated payroll / personnel system. When implemented, this system will replace the NRC Payroll System and Automated Personnel System currently running on Data General minicomputers at NRC.

An Interagency Agreement between NRC and HHS, covering planning and startup activities for the payroll / personnel cross-servicing effort, was signed on Decemaer 31, 1992.  ,                                             Office of Personnel Items of Interest Week Ending January 15, 1993 Child Development Center Initiatives Mary Louise Roe, Organizational Development and Training (0P), and Office of Consolidation (CONS) staff, Jean Bouquet and Joann Manzek, met with the Government Services Adninistration (GSA) on January 13, 1993, to review the                        -

interior and exterior designs for the Child Development Center. On January 14, 1993, CONS staff also gave a presentation on the status of the center for members of the Joint Labor-Management Child Care Committee and representatives from the federal Women's Program Advisory Committee and the Office of the Executive Director for Operations. Ethics Orientation Trainino_ Offered at Headauarters On January 13, 1993, NRC began presenting the first of its 2-hour Ethics Orientation, as required by the Office of Government Ethics. The course is presented by the Office of the General Counsel and the Office of the inspector General. The training will continue until May 6,1993, and will be of fered at the regions and at the Technical Training Center, Chattanooga, during this timeframe. The Ethics Orientation is mandatory for individuals in specified positions. hmervisina Human Resources Course Held in Regjon Ly The Supervising Human Resources course was held in Region IV from January 11-15, 1993. Office of Enforcement Items of Interest Week Ending January 15, 1993 Sionificant Enforcement Action A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $ 10,000 was issued on January 11, 1993 to St. Clares Riverside Medical Center, Denville, New Jersey. The action was based on violations of NRC requirements which, in part, included failure to instruct persons caring for a patient with brachytherapy implants and failure to implement the radiation safety and quality management programs. These violations contributed to a patient mi$ administration and an un)lanned exposure to the hand of a licensed practical nurse at the facility. T1e violations contributing to the misadministration were categorized at Severity Level Ill and were identified by HRC, therefore, 50 percent escalation of the base civil penalty was warranted. 1. Wolf Creek Nuclear Operating Corporation (Wolf Creek) paid the civil penalty in the amount of $50,000. The action was based on a violation of the licensee's work control programs which resulted in the degradation of the essential service water systen. (EA 92-191)
2. Wisconsin Electric Power Company, (Point Ber.ch Di.'t 2) paid the civil penalty in the amount of $75,000. The action was based on the inoperability of one train of the safety injection system piping in the recirculation mode due to the presence of foreign material in the piping. (EA 92-205)
3. New York Power Authority (Indian Point 3) paid the civil penalty in the amount of $137,500. The action was based on the failure of the licensee to provide complete and accurate information to the NRC during an enforcement conference and on the failure to obtain adequate medical assurance of an individual's fitness-for-duty before returning him to licensed duties and the f ailure to establish a follow-up program for that individual and another individual who had tested positive for drugs. 4. University of Michigan, Ann Arbor, Michigan, paid the civil penalty in the amount of $3,750. The action was based on two violations of HRC requirements and include the failure to perform proper surveys which resulted in radioactive contamination in the public domain and the failure to perform the necessary survey of unrestricted areas.

l i Office of Consolidation Items of Interest Week Ending January 15, 1993 Sn onLBuildina Status The elevator motors, cables, and platforms (but not cabs) are installed and , operational on temporary power. Frames and outer doors for elevator lobbies are installed from D Level through Floor 5. Exhaust fans are all installed on parking levels. Each of the elevator cores from Floor- 2 through 10 is now enclosed with drywall. The remainder of the core walls (e.g., toilets, telephone rooms, mechanical rooms) with the exception of Floors 4 and 5 have passed "close-in" inspection by the county. Dry wall construction enclosure of the lobby. is 90 percent complete. Plaza veranda heating units and ceiling grid are installed. Concrete has been poured for northwest plaza stairs to Level A, steps at TWFN main plaza entrance, gazebo steps and northernmost steps from pedestrian concourse up to plaza lawn. Handrails have been installed for Stairs #4 and #5, Floors 2 through 10. On January 12, the interior design contractor, DBI, presented proposed fabrics, finishes, colors, and lighting plans for the Emergency Operations Center to AE00 management. The HVAC/ Electrical design contractor, LDE, took IRM staff to see . computer rooms containing the type of HVAC equipment proposed for the TWFN Computer Center. On January 13, at the weekly progress meeting with Lerner/GSA/NRC, Lerner announced.that construction drawings for interior build-out will be available from the interior design firms, 081 and LDE, on February 3. On January 14, the interior design firm, DBI, presented fabrics, finishes, colors and design changes to Consolidation management and NRC Day Care Committee members. 9 Office of State Programs Items of Interest Week Ending January 15, 1993 Meetina with New Jersev Officials i Region I staff met with officials from the New Jersey Department of Environmental Protection and Energy on January 12, 1993, in Princeton, New Jersey. The purpose of the meeting was to provide an overview of the New Jersey radiation protection and nuclear engineering programs and the NRC's reactor safety and regional inspection programs. Follow up items from a previous meeting included the status of the low-level radioactive waste storage inspection memorandum of understanding, the emergency response data system memorandum of understanding and training. The State: representatives . indicated an interest in NRC's inspection activities-on individual plant examinations. The meeting ended with a commitment of continued cooperation between the New Jersey and NRC. Meetina on_pravan Sit.g Dennis Sollenberger, State Programs, participated in a ceeting on the Uravan site with representatives from the Department of Energy (DOE), the State of Colorado and other NRC staff on Jenuary 12, 1993, in Albuquerque, New Mexico. The purpose of the meeting was for DOE, through its contractors, to present the results of a limited review of the reclamation plan for the Uravan-tailings disposal area. This tailings disposal area is being considered for the disposal of Title I tailings from the Naturita site. Several unresolved issues were identified. 00E will issue its report and the. State of Colorado will address the unresolved issues. Federal Water Resources Su_bcommittee , Maria Lopez-Otin, Federal Liaison, and Tom Nicholson, Office of Research, attended a Federal Water Resources Subcommittee meeting en Ja%ary 13, 1993. There were two formal presentations at the meeting. The first, by the Environmental Protection Agency, was on EPA's Environmental Monitoring Assessment Program (EMAP). The EMAP is.a national. survey program which- [ provides a framework for monitoring various' ecological systems and identifying trends to aid-in designing preventive measures. The EMAP is an EPA interagency program which includes NOAA, USGS, 00E, NASA, BLM and others. The second presentation was from Mr. iiicholson, who discussed the NRC's-  ; research program focusing on hydrology. Mr. Nicholson gave details of ongoing. research projects in high-level and low-level radioactive waste, such as the.

1. Jersey Shore Medical Center Enforcement Conference 1

On January 14, 1993, a closed enforcement conference was conducted with Jersey Shore Medical Center, Neptune, NJ. The conference discussed a misadministration at the facility that occurred October 28-November 3, 1992, wherein a patient being treated for lung cancer received 300 rads i per day instead of the prescribed 180 rads per day. The misadministration resulted from an incorrect calculation being performed by the radiation technologist. .The licensee discovered the error, and made appropriate changes to the treatment-schedule, so that they believe there is no impact on the patient _or patient treatment.

2. Millstone Northeast Utilities restarted Millstone Unit 2 over the weekend from a refueling and steam generator replacement outage which began on May 29, 1992. The reactor was taken critical at 1:40 a.m. on Sunday, January 10, 1993, after the successful completion of a-reactor coolant system hydrostatic test on 1/8. The restart was observed by a team of inspectors from Region 1. Physics testing of the reactor core continued until noon on Monda), January 11, then the reactor was taken to hot shutdown to complete other planned testing; the return to critical operations occurred late Monday.

1. Carolina Power and Licht _ Company On January 14, the Regional Administrator, accompanied by Regional staff, was at the Brunswick facility to present the latest SALP results to representatives of Carolina Power and Light. Company and also held a public meeting with local officials attending the SALP presentation.
2. Tennessee Valley Authority - SeouoVB,h On January 13, representatives from the Tennessee Valley Authority -

Sequoyah Nuclear Plant were in the Region 11 Office to attend an Enforcement Conference. The Conference discussed the results of a recent inspection which identified violations for failure to control safeguards materials. Region III Items of Interest Week Ending January 15, 1993

1. Commonwealth Edison Comoany On January 15, 1993, A. Bert Davis, Regional Administrator, and other members of Region 111 senior management, met with Messrs.

J. J. O'Connor, Chief Executive Officer, Cordell Reed, Senior Vice President, M. J. Wallace, Chief Nuclear Operating Officer, and L. O. DelGeorge, Vice President, Nuclear Oversight and Regulatory Services. The meeting was held to discuss topics of mutual interest such as the recently named LaSalle Station Site Vice President, status of the corporate reorganization and the Integrated Management Action Plan, effectiveness of the site Quality Verification organization, and corrective actions for recent personnel errors at Zion Station.

2. On January 14, 1993, Commonwealth Edison Company announced that Warren P. Murphy will be the Site Vice President for LaSalle County Station.

Mr. Murphy has over 30 years of nuclear and utility experience and is currently the Senior Vice President of Operations for the Vermont Yankee Nuclear Power Corporation. He will assume the new position on February 3, 1993.

3. Overhoff Technoloav Corooration On January 12, 1993, an enforcement conference was conducted with Overhoff Technology Corporation. Multiple violations were discussed including the possession of radioactive materials in excess of licensed limits and the use of materials for purposes not authorized by the license.
4. Pike Community Hosoital On January 14, 1993, an enforcement conference was conducted with Pike Community liospital to address their corrective actions for multiple violations identified during a recent inspection. While individual violations were not significant, the multiple viointior.3 demonstrated a lack of control over the program. Escalated enforcement is being considered.
5. GA0 Audit Recardina Chemetron GA0 auditors visited the Region on January 14, 1993, to discuss remaining questions relating to NRC oversight of the decommissioning of the Chemetron site. They plan to complete the report of the survey phase of their review by April 1993.
6. Commissioner Curtiss Visit During January 13-15, 1993, Commissioner Curtiss, accompanied by HQ and Regional Staff, visited Allied Signal, Paducah Gaseous Diffusion Plant, JANUARY 15, 1993 ENCLOSURE R

The Director, Division of Reactor Projects and selected members of the Operational Safety Team Inspection (OSTI) conducted an exit meeting on January 12, 1993, with the licensee at STP. The inspection was conducted in order to provide Region IV staff with a better understanding of plant operations and the manner that different licensee organizations supported plant operations.

2. Manaaement Meetino at South Texas Proiect (STP)

The Regional Administrator: Director, Division of Reactor Projects; and , other members of the Rcgional staff net with STP management on - January 13, 1993. The purpose of the meeting, at the licensee's request and open to attendance by members of the general public, was to discuss . initiatives taken by the licensee to improve performance at STP. l
