The inspection included a review of activities

author,

for your South lexas Project, Units 1 and 2, facility.

At the

conclusiva of the inspection, the findings were discussed with those members

of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the ieport.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observations of

activities in progress.

During this inspection, two unresolved items were identified.

The first

unresolved item (Section 2.1) pertains to several instances in which two

chemical operators apparently f alsified their log sheets and were subsequently

dismissed.

This issue will remain unresolved pending further f4RC review.

The second unresolved item (Section 2.2) pertains to six problems and events

that stemmed from plant personnel conducting work activities associated with

the wi any components, the wrong system trains, or the wrong unit.

A similar

ever,t was identified as an unresolved item in NRC Inspection

Report 50-498/92-29; 50-499/92-29.

A meeting between NRC and 11ouston Lighting

and Power Company will be scheduled in order for NRC to gain a better

understanding of the causes of these personnel errors and the actions you have

taken or planned to prevent recurrence.

The

schedule for this meeting will be established by separate correspondence.

During this inspection period, your staff identified that selected Technical

Specification surveillance procedures were less than adequate (Section 2.6).

Although this constituted a violation of f4RC requirements, the violation is

not being cited because the criteria in Section yll.B.2 of the f4RC's

inforcement Policy were satisfied.

This violation was f ound during a review

that was committed to as a correttive action for a previous licensee Event

Report, and your staff took prompt and effective actions to correct the

problem.

Although the discovery of the surveillance deficiency was positive,

we note that this was the third example in recent month; 'n which the

discovery of inadequate surveillance procedures resol'ed in one or both units

being placed in Technical Specificat ion 3.0.3.

It is our understanding that

when you complete the corrective actions associated with the review of all

~

reactor protection system and engineered safety feature surveill;nce

procedures, you will niake a determination as to whether the scope of the

procedure review should be expanded to encompass other types of surveillance

procedures,

in accordance with 10 CFR 2.79L of the liRC's " Rules of Practice," a copy of

'his letter and its enciasures will be placed in the f4RC's Public Document

ivam.
