Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:

Nonconforming Terminations in CVI Corporation Equipment (SD-75)

In a February 13, 1985 telephone conversation between your Mr. T. Rebelowski and our Mr. R. K. McCarthy, Northeast Nuclear Energy Company reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e).

The potential significant deficiency involved improper terminations found in equipment supplied by CVI Corporation. Termination deficiencies include crimps which were unacceptable per vendor's instructions, terminations crimped with the wrong tools, multiple conductors crimped in one lug, and incorrect lug size used.

The following equipment has been inspected and found to be deficient at Millstone Unit No. 3:

o HVQ* ACUS 1 A, IB,2A, IB - Engineered Safety Features Building Self-Contained Air Conditioning Units o HVR*PNLFLT J A, IB - Auxiliary Building Filtration Units o HVR*PNLFLT 2A,2B - Fuel Building Filtration Units o HVR*PNLFLT 3A, 3B - Supplementary Leak Collection and Release System Filtration Units We have determined that this condition represents a significant deficiency in the final design of Millstone Unit No. 3.

At Northeast Utilities Service Company's request, our Architect-Engineer, Stone

& Webster Engineering Corporation, has performed a review of the Vendors QA.

and Engineering Records and found no pertinent information regarding lug crimping. Nonconformance and Disposition Reports 10916, 10828, 10996, 10926, and 10893 document inspection and rework of these deficient lugs. A 100%

inspection of the lugs in the 2 remaining CVI panels (3 HVC*PNLFLT 1 A and IB) will be performed.

We will provide you with an update on this matter by August 8,1985.
