STATUS REPORT FROM THE NRC TEAM ONSITE AT DAVIS BESSE Atapproximate}y$:00pmJoday,E.Rossireportedthefollowingprogress:

Yesterday's activities consisted of introductory meetings with the Plant Manager-and the Assistant Vice President of Toledo Edison. Next the utility gave a briefing on the event sequence.

It became clear that the initial written description of the event was incomplete and that a number of potentially important details were not included. As a result, the licensee is redoing the sequence of events.

The team discussed with the Operations Superintendent the design of the Davis Besse plant in terms of the systems that were involved with the event. The event itself was not discussed at this point. The team was also briefed by Donald Kosloff, the Resident Inspector, regarding his knowledge of the plant event and associated actions by the licensee.

l The team had a discussion with the licensee regarding the current status of equipment quarantine and will have further discussions regarding what equipment is being quarantined and on what basis, and what other equip-ment was not quarantined and again on what basis. The team is maintain-ing a close communication with Regional personnel with regard to the i

subject of equipment quarantine and procedures for release of the equip-l ment. At this time, these procedures are focusing on assurance of adequate control and documentation of the "as-found" condition in order l

to focus on " root cause" analysis.

Todays activities consisted of two interviews between the team, the j

Shift Supervisor, and the Assistant Shift Supervisor.

These interviews totaled approximately 3} hours. The interviews are going slow and the team believes there are 6-9 additional personnel involved in the event that should be interviewed. All interviews are being transcribed via l

court reporter.

.. The significance of this event is becoming clearer. One aspect is that there were multiple equipment failures which may or may not have been independent.

Further, there is evidence of personnel errors. The actual description of the event is considered to be known with reasonable accuracy although it could change some upon further analysis.

- Overall, the licensee is considered to be candid, forthcoming, and cooperative in their discussions with the team.

The licensee currently has two lawyers monitoring the team's activities; a local representative of the company and a Washington lawyer (J. Silverberg). The team gave the option to the people being interviewed as to whether they desired a lawyer and/or a company representative present. This morning the Shift Supervisor elected to have no one else present while the Assistant Shift Supervisor requested the presence of Mr. Silverberg and the Operations Superintendent (Phil O' Conner). There was no interference or difficulty associated with these representatives.

Overall, the team's activities are going slowly, and it is estimated that in order to gain information on the status of plant equipment, the team will have to remain through the weekend.

The team was aware that there was a 2 o.r 3-person B&W transient assess-ment team onsite, together with three INP0 employees (Bill Lavalle, Bill Brown, and one other). The team indicated that they had no interaction or interference from the activities of the B&W and INP0 teams (I noted that Steve Rosen, Vice President and Director of Analysis and Engineering at INP0 is traveling to the site to arrive tonight).

I also noted that there is Congressional interest in this event and that Henry Meyers and Richard Udall have requested an oral briefing from Region III this afternoon.

Consequently, I requested that the l

team, as soon as practical, forward the most current sequence of events associated with the transient.
