STATUS REPORT FROM THE NRC INCIDENT INVESTIGATION TEAM AT DAVIS BESSE At approximately 5:00pm on Monday, June 17, 1985, E. Rossi reported the following progress:
1.
The NRC team interviewed the three equipment operators that were on duty during the event and were dispatched from the control room to return the AFW pumps to serv' ice. The focus of the discussions was on the step-by-step account of the actions necessary to get these pumps back into service.
In this regard, there was a concern regarding the key-locked doors, and the possibility that the cards could break off in the doors which would result in delays in getting to the equipment.

The most recent licensee's sequence of events was being reviewed by the l
team, and a meeting is scheduled for today, June 18, to resolve and clarify a number of items. The focus at this time is on the. key events that directly affected the transient. Other aspects of the sequence t
related~ to associated events that occurred during the transient will be reviewed and studied in greater depth during the team's root cause l
analysis. The licensee indicated that the B&W and INP0 teams agree with the licensee's sequence. The NRC team also believes that the licensee's sequence is fundamentally sound but that questions remain as to the l
precise sequence that some equipment actuated; for example, which of the MSIVs closed first.

With the assistance of ELD (S. Burns), a set of groundrules for the review of transcripts was developed and given to the licensee.
Provisions are being made for the transcripts to be controlled by a single individual and to allow participants in the discussions to have access to their portion of the transcripts in order to make any necessary corrections.
Note-taking will be permitted, but no copies of the trans-cripts will be made at this time.
All transcripts will be released to the licensee, public, and other NRC personnel at the same time.

William J. Dircks Provisions have been made for NMSS review of the transcripts with regard to a potential need for protection because of security or safeguards information.

The team has agreed with the licensee's troubleshooting procedure for investigating the causes of equipment failure and/or malfunction. The licensee is controlling the schedule for the equipment investigation and is proceeding very slowly and cautiously. There are no known delays in these activities because of the team's activities. The licensee is pre-paring an action plan for the key components.
Prior to implementation, this plan will be reviewed with the team.
It is highly unlikely that extensive failure analysis will be available this week.

E. Rossi indicated that he will telefax to me copies of the groundrules on the handling of the transcripts and the licensee's procedure for troubleshooting. Further, in order to promote communication from the team, we tentatively ~ planned a conference call between E. Rossi, W. Dircks, H. Denton, J. Taylor, J. Keppler, and myself for Wednesday afternoon, June 19, perhaps around 3:00pm.
I am to confirm the specific time and date for this call.
