SEALED SOURCES In May 1984, this office was notified by one of our licensees, the Fedaral Emerger.cy Management Agency, that several of their 3M sealed sources had been reported to them as leaking. The licensee submitted a written report of this incident to this office by letter dated June 25, 1984 (copy attacned). An inspection of this licensee was performed on December 19, 1984. The inspector obtained a copy of a draft licensee report which provides additional information abcut the leaking sources. We are attaching a copy of the draft

licensee report.

Since 3W is located in Region III, we are sending you copies of these reports for your possible use during a future inspection of that licensee.

Dear Mr. Davis:

On May 17, 1984, I called and notified you of a leaking sealed source in a CDV-782 soarce set issued to Rhode Island. I agreed to furnish you a report when the investigation was completed.

On Jane 13 I called to notify you of the leaking sources in Texas and Massachusetts and the actions that FEMA had taken to date. Since this is no longer an isolateo incident, you requested an interim report.

The enclosed interim report sumarizes the incident to date and what actions have been taken.

Mr. John Hickey, Office of Nuclear Material Safety and Safeguards, called on June 22 concerning the incident. I agreed to furnish him a copy of the interim report.
