This refers to Certificate of Compliance No. 4909 for the Model Number GE-21PF-1 packaging which has an expiration date of February 28, 1993.
To ensure continued use of this package, you should submit an application. for renewal of the certificate at least _30 days prior to the expiration date. The sections on operating procedures, acceptance tests, and maintenance programs should be reviewed to assure they are complete and current.
