Entergy Operation, Inc. hereby submits the response to the exercise weakness identified during Inspection 50-416/92-23.

The exercise inspection report identified a general weakness in our ability to provide clear and accurate emergency notification messages to state and local agencies.

Several examples were provided which were minor when considered individually, however, in aggregate they indicated a general weakness in tnis area.

The major contributor to this weakness is the form used to generate and issue emergency notifications to tho state and local agencies.

A single form is used that contains areas for information covering all emergency classifications. The current format of the form is conducive to personnel errors when completing the form prior to an emergency notification.

Efforts are in progress to review and revise the emergency notification form from a human factors perspective. The form must be simplified to ensure all required information for the existing conditions is identified by the preparer and transferred to the form. The goal of this offort is to produce a form that can be filled out quickly and accurately and contains all the required information for the current conditions.

which reported an escalation of the emergency classification, was not transmitted in a timely manner.

The notification was not transmitted until approximately 20 minutes after the escalation occurred.

During the preparation of the notification several evolutions were.
