RESPONSE TO INSPECTION REPORT 84-27 One item of noncompliance was identified in Inspection Report 50-255/84-27 dated January 31, 1985. The following is our response to this item:

Noncompliance (50-255/84-27-01)

Technical Specification 2.3, Table 2.3.1 states that a low flow trip setpoint of greater than or equal to 95% of full flow is required for four primary coolant pump operation and a setpoint of greater than or equal to 71% is required for three pump operation.

Contrary to the above:

For the time of operation during July through September 1984, all four a.

channels of four pump low flow trip were found to be less than the required 95% setpoint.

(The greatest calculated error was 0.63%.)

b.

For an indeterminate period of time, possibly since plant startup, the three pump low flow trip was set less than the required 71% setpoint.

(The greatest calculated error was 2.9%.)

Corrective Action Taken and Results Achieved The four-pump low-flow-trip has been correctly adjusted to the required limit.

The three-pump low-flow-trip has been set at a high level such that three-pump operation is precluded.

' Corrective Action Taken to Avoid Further Noncompliance

The low-flow-trip surveillance procedure will be revised to ensure that-supporting tests are completed, and updated trip setpoints are utilized. A

.. corrected calculation method will be determined for the three-pump low-flow-

. trip. Procedures will be revised to include the corrected calculation. The three-pump low-flow-trip will be adjusted to allow three-pump operation following determination of the proper setpoint.

All'special test procedures will be reviewed to determine those. tests that will be repeated in the future. ~ Additional actions to ensure performance of the special~ tests will be determined as appropriate, based on the results of-

~this review.

-adjusted to be conservative with respect to Technical Specification require-ments.- Additional actions to ensure continued compliance will be completed by September-30, 1985.

Additionally, the Inspection Report cover letter specifically requested that we address what actions we plan to take in the area of surveillance to assure "the adequacy of scheduling; data-review for acceptance and applicability; and post refueling outage review to ensure,all required special tests have'been completed".- Our response to these concerns are as follows:

.The improper trip.setpoints of this event resulted from a failure to perform a special test procedure. These procedures are provided to administrate testing that-may occur at infrequent intervals or for special conditions that will not be repeated (ie, initial fuel load tests). A special test procedure is utilized to determine the correct setpoint for the low-flow-trip following each refueling. A surveillance test. procedure is used to. implement the results of that special test.- Recurrence of this-type event will be precluded.

Since the special test was not performed, and a reference to the test was not made in'the surveillance' procedure, updated trip values'were not determined.-

Palisades Plant Response IEIR 84-27 March 4, 1985 tests, very few are currently active. A review of these tests will indicate if a similar occurrence may be likely, and appropriate action to ensure scheduling and performance will be provided. This review will be completed by September 30, 1985.
