NRC INSPECTION REPORT N0. 50-498/92 24; 50-499/92-24 (NOTICE Of

' VIOLATION)-

1his refers to the inspection conducted by Mr. J. 1..Tapia and other members

of this office.during the period July 5 through August 1, 1992. The

inspection included a review of activities authorized for your South Texas

'

-Project, Units 1 and 2.

At the conclusion of the . inspection, the findings

were discussed with those members of your staff identified in the enclosed

report,

Areas examined during the-in'spection'are identified in the report. Within

'these_ areas, the inspection consisted of selective examinations of procedures

,

~

and' representative records, _ interviews with' personnel,-and observations of

activities in progress.

An1 unresolved item is tidentified in Section-4.3-of the enclosed report.-

Additional ~ inspection followup -is: required in order to determine whether

dCtions taken to date have 'been sufficient .to address'severalstemporary

L

modifications ~ that are greater than 2 years old.

-On the basis of the results of.thisLinspection, certain of your activities

appeared to be in violation of NRC requirements, as specified .in.the enclosed -

Notice of Violation'

Violation A of this Notice discusses one

-example 'of. an ' inadequ(Notice) . ate cystem restoration, which was caused by a pro

,

that was not appropriate:to the circumstances. This' violation'is of concern

-

.because the condition (as! described in Section 3.2.3 'of the enclosed report)-

was NRC. identified and represented a failure of a licensed reactor operator to

-

recognize a procedural deficiency which, under the circumstances identified,-

resulted in the; control. room envelope not being fully restored to its _ normal

-

configuration before a surveillance-was signed off as being satisfactorily

completed.. Violation B~ pertains to inadequate corrective actions _ for design

and procedural problems associated with essential chilled water ficw switches.

!These-problems: were identified several months ago.

This violation (identified

in-Section 3.2.1 of the enclosed repert) is of concern because it appears that

little or no effective corrective actions would have been taken had NRC not

performed a reinspecticn of this issue.

.You are required to respond to this letter and should follow the instructions

specified in the' enclosed Notice when preparing your response,

in your

response, you should document the specific actions taken and any additional

actions you plan to prevent ' recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy

of this letter and its enclosures will be placed in the NRC i Jblic Document

Room.

The response directed by this letter and enclosed Notice are r.ot subject to

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, PL 96-511.
