SUBJECT:- NRC INSPECTION REPORT NO. 50-459/92-26 (NOTICE OF VIOLATION)

This refers to the inspection conducted by Messrs. E. J. Ford and D. P. Loveless
of this office on July 5 through August 15, 1992.
The inspection included a
review of activities authorized for your River Bend Station facility.
At the
conclusion of the inspection, the findings were discussed with those members of
your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within these
areas, the inspection consisted of selective examination of procedures and
representative records, interviews with pusonnel, and observations of activities
in progrecs.
Based on the results of this inspection, certain of your activities appeared to
be in violation of NRC requirements, as specified in the enclosed Notice of
-
Violation (Notice).
The violations are of concern because they both had the
. potential to affect the operability of the automatic depressurization system.
We are particularly concerned that maintenance activities, which were considered
to be outside the skill of the craft, were not governed by appropriate
proedure3.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. in your response,
you should document the specific actions taken and any additional actions you
plan to preven recurrence.
After reviewing your response to this Notice,
including your proposed corrective actions and the results of future inspections,
'
the NRC will determine whether further NRC enforcement action is necessary to
ensure compliance with NRC regulatory requirements.
In addition to the violation discussed above, a violation was idantified that
involved the improper posting of a radiological controlled area.
The area was
posted with signs that Indicated that the area was a radiation area and also a

the criteria specified in Section VII.B.1 of Appendix C to 10 CFR Part 2 in that
the violation was of minor safety significance and the licensee took prompt
-actions to correct the posting problem.
An unresolved item was also identified during this inspection period.
The item
involved the adequacy ur the procedural controls implemented for mad 'anance on
an emergency diesel generator and the adequacy of the actions taken ts correct
a problem with an emergency diesel generttor.
These issues remain unresolved
pending a review by a Region IV specialist,

in accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosure will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Menagement and Budget, as required
