& Associates steam generator (S/G) FOSAR activities at Unit I serve as the basis for the evaluation and assessment of the occupational radiation exposure to nessrs. K lG 3 Z [$$1 M for the date of incident, September 28, 1989 and the trenitorthfFEFTbd.

STATE!!EtiT OF SITUAT10!1 The relocated whole body TLD, which was worn on the right arm just above the elbow during the S/G secondary-side handhole work on September 28, 1989, did not remain at the initially attached location for two of the workers. This created a situation in which the TLD processing result would not represent the total whole body dose for the affected workers.

DATA RESOURCES FOR DOSE ASSESSt!EllT Radiation exposure for the whole body was assessed using a combination of information gathered for the incident report such as worker extremity and whole body TLD processing results log book entries for FOSAR work: Radiation Technician and FOSAR work crew statements; measurements and arm position testing using the full scale mockup of the tube sheet area, handhole and steam generator shell: special irradiation tests of TLD material on a mannequin arm as i n s e.r t e d in the 'B' steam generator hand hole number 21 and written records of incremental changes in the pocket dosimeter readings for dose tracking.

DECIS10t4 LOGIC The following logic was used to substantiate our case for the dose results which are listed in FillAL D0SE RESULTS section:

1. Extremity TLD processing data is considered valid for all FOSAR workers.

c. An inspection of the TLD dose generation report and each TLD element result indicates the information does not contain unusual data nor do element-to-elemant variations and ratios appear unexpected for this type of work.

3. The dose rate at the location just above the elbow (upper arm) for a full arm insertion was determined using an irradiation test with Panasonic UD812AS2 TLDs (refer to Figure 1). The dose rate at the location just above the elbow was:

A review was conducted of each worker's f1RC form 4 equivalent for the applicable expos"re monit oring period. Each flRC Form 4 equivalent was a complete radiation exposure history with record dose entries. This permits the quarterly whole body dose of 3 rems to be the applicable dose limit providing the available 5(ll-18 ) dose bank _ does not cap the permissible quarterly limit of 3 rems. Each worker had well over .1 rems in the remaining 5(n-18) dose bank. Based on __ t his_ review and the above assessments. no regulatory dose limits were exceeded.

The current state of the termination occupational radiation exposure reports (fiRC form 439) are presented as Attachments 1 and 2.

This letter will be attached to an RCH form 4.4 Section 2- "Do s irne t ty Inconsistency / Dose Assessment Report" for Hessrs.y [ y nd filed with each worker's occupational radiation exposure history itle in the ERC.-

STEAM GENERATOR SECONDARY SIDE HANDHOLE (F.O.S. A.R. )

Immediate Corrective Actions

1. All steam generator handhole foreign object search and retrieval (FOSAR) work was terminated.
2. A critique was held regarding this issue. In attendance were the vendors working on the job, the vendor's management, appropriate Radiological Control personnel (including the Radiation Technician covering the job),

and the General Manager of Nuclear Operations.

3. A special meeting was held with all vendor and Level 3 and 4 DLCo nanagement personnel who had personnel reporting to them for outage support. The meeting was chaired by the General Manager of Nuclear Operations. The topic of the meeting was the significance of supporting the Radiological Control Department and specifically obeying all requests by the Radiation Technicians.
4. A survey was performed in the vicinity of the top of the tube sheet where the individual had his hand positioned.

NOTE: The steam generator secondary handhole mockup was utilized to develop additional survey techniques applicable to this situation.

5. A survey was performed in the area of concern with respect to whole body exposure (above the elbow).
6. A verification' survey was performed to ensure that no conditions had changed in the steam generator secondary haadhole. This was done to verify that the dose rates initially used by the Technician had been appropriate.
7. Radiological Control Manual. Chapter 3. Rp 8.1, was revised to incorporate -

a tracking mechanism for extremities when working in the steam generator secondary handholes.

8. A TLD tree was utilized to establish a dose gradient inside the 'B' steam

-generator handhole.

9. Modified the Radiological Work Permit to incorporate specific _ corrective actions.
10. The vendor by memo to the General Manager, Nuclear Operations, acknowledged-that they will comply with controls as discussed at the critique,
11. Director of Radiological Operations issued a letter to all _ Radiological Control foreman and outage coordinators emphasizing the Radiation Technician's stop work authority.
