This is to infcrm the commission that a Notice of Violation and i

Proposed _ Imposition of Civil Penalties in the amount of $12,500 will be issued on or aoout September 2, 1992 to CTI, Inc.

This action is based on the licensee's failure to 1) use a separate alarm rateme.er, 2) conduct complete radiation surveys, 3) post high radiation areas, 4) conduct a program for inspection and maintenanca of radiographic exposure devices, containers, and l

source changers at interv8.ls not to exceed three months, 5)' hold a certificate of compliance to transport Type B radioactive packages, and 6) have an NRC approved quality assurance program l

for transport of Type B packages.

Violations'1 and 2 were y

aggregated as a Severity Level III problem.

Based on willfninese violation 3 was escalated to a Severity Lev 31 III.

The ocher.v.olatio-were categorized at severity levels IV and V and not as'essed a civil penalty.

The base civil.pe:.alties were each escalated 50 percent since they were identif3Sd by the NRC.

The penalty for the Severity Level III problem was mitigated 50 percent for prompt and aggressive corrective actions.

i It should be noted that the licensee has not been specifically informed of the enfo.rcement action.
