Calgary, Alberta, Canada T2L 1G8 Subject: REQUEST FOR LICENSE AMEhDMENT FOR STORAGE ONLY Dear Mr. lieiken: We have considered your request for a " Storage Only" amendment to your radioactive materials license including discussions of your request with NRC Headquarters. Based on our review, there appears to be three plausible options with regard to your radioactive materials license. 1. Obtain a suitable lccation for stcrage of racioactive materials where the materials would be under the control of irdividuals who are trained in handling radioactive materials and able to ensure the materials are kept secured aad the public is protected. Once you have obtained a suitable location for storing radioactive rr.aterials we would grant an amendment for storage only. Prior to licensed material use, an additional amendment would be required. 2. Maintain your current license for use at temporary locations anywhere in the U.S. where the NRC maintains jurisdiction for regulating the use of licensed materials. This option would require that all fees be maintained at current levels unless you can satisfy the new criteria for small entities. Copy enclosed. 3. Terminate your present license and submit a new application when the opportunities for employment improve. Please examine the options described above and provide us with further information regarding what course of action you prefar.

Inc. of Ant orage shipped two Amersham Model 660 devices to Amersham in early 1991. Each device contained a source for disposal. The devices belonged to Ultra Techn logy, but not the satirees.- hence Amersham sent leak tests to CTI. At a later date. Ultra Technology Inc. sold these devices to WesTest Inc. 8410 Wee Mercer Way, Mercer Island, Washington 9S040, Amersham shipped the devices, loaded witti new sources I assume. The device Serial Numbers are 4281 and 3:24. CTI shipped three emp:y 660 devices to me in Canada. I asked (by telephone) if. l they had been leak tested and was assured that they had been and since the sources had l been transferred to CTI, I had no claim on the leak tests. ' L ~~ l received Serial Numbers 4850,3433 and 3543 and subsequently sold S/Ns 3433 nr.t. l 3543 to Rocky Mountain Inspection Ltd.,59 Woodvalley Mews S.W. Calgary, Alberta i 2W , ! SN4. S/N 4850 is in storage in this company's source storage vault. LTI shipped our 865 device containing a source to Amershata. After the source was removed I had it shipped to a premises I own in Utah where it p presently stored in locked storage.
