o Office of Nuclear Reactor Regulation items of Interest Week Ending January 1, 1993 Survey of Potential for BWR Strainer Blockaae (SCSB)

On July 28, 1992, the Barsebeck plant in Sweden experienced an event in which fibrous insulation in the drywell was dislodged and became trapped on the ECCS and spray system suction strainers, in an effort to determine the applicability of the Barsebeck event to U.S. reactors, NRR has initiated a request to the five regional offices to compile the characteristics of all BWR plants with respect to their vulnerability to strainer blockage under accident conditions.

Specifically, the request addresses the type (s) of insulation used in the drywell, the number and type of relief valves that discharge to the drywell, number and size of strainers, pump flow rates and the characteristics of alternative water sources.

It.is requested that tne survey be conducted by the resident inspectors within a two-week time frame.

Thermo-laa Test Results for TVA TVA conducted a series of Thermo-Lag fire barrier tests at Omega Point Labs in San Antonio, Texas on December 21 and 22, 1992. The first test, conducted on December 21, 1992, consisted of 5 inch conduits, 1 inch conduits and air drop assemblies.

The installations of these assemblies were enhanced from the vendor's recommended installation. Test results indicated less than 350*f on the large conduit and no post hose stream damage.

The 1" conduits and air drops recorded high temperatures indicative of burn through.

Post hose stream inspections noted burn through in several locations.

Currently TVA only considers the 5 inch conduits as passes The test performed on December 22 resulted in thermocouple anomalies due to moisture in the conduits, and TVA considered the results indeterminate.

TVA plans to develop new test assemblies and instrument them with teflon-coated thermocouples to continue qualification tests for the Thermo-Lag barriers in January 1993.

Feedback on Meetina of NUMARC AD H0C Advisory Committee on Protective Measure

_Reouirements ORSS:PSGB staff were provided a-summary of the results of a December 22, 1992, meeting of the NUMARC Ad Hoc Advisory Committee on protective measure requirements.

One purpose of the Ad Hoc Committee meeting was to discuss changes or alternatives to their positions expressed.in a June 24, 1992, letter to the NRC. The June 24, 1992, letter provided a proposal _ for an alternative protection strategy for protection against the' insider threat.

Staff was informed that the committee did not identify any alternatives or changes to their original proposal.

JANUARY 1, 1993 ENCLOSURE A

Office of Nuclear Material Safety and Safeguards items of Interest Week Ending January 1, 1993 Inventory Chance Report The Nuclear Regulatory Commission received from the Nuclear Materials Management and Safeguards System the inventory change reports of the five U.S.

low enriched uranium fuel fabricators that report to the International Atomic Energy Agency (IAEA). The report consists of all transactions conducted for the month of November 1992 that had an effect on the operators' inventory.

After review from a member of the International Safeguards Branch, this report will be transmitted to the IAEA for nuclear material accountability comparison with the facilities' ledgers. The five reporting facilities are General Electric Company, Westinghouse Electric Corporation, Siemens Power Corporation, Combustion Engineering Company and B&W Fuel Company.

Paducah Site Visit The Nuclear Regulatory Commission staff from Region 111 and Headquarters visited the Paducah Gaseous Diffusion Plant site on December 16-18, 1992, for briefings and discussion of safety and safeguards information. Discussions covered planned enrichment uprating, earthquake effects and emergency planning, criticality prev]ntion, fire protection, radiation protection conformance, material control and accounting conformance, and security conformance.

Office of Civilian Radioactive Waste Manaaement (OCRWM) Ouality Assurance (0A)

Submittal On December 21, 1992, the Department of Energy (DOE) requested that the Division of High-Level Waste (HLWM) review and accept its " Quality Assurance Requirements and Description" (D0E/RW-0333P QARD) for OCRWM. This document is the highest-level QA document for the civilian radioactive waste management program, and it supersedes all previous QA program commitments.

Overall, it contains the requirements that DOE and its contractors must meet in the development of their individual QA programs.

The document was developed to meet Nuclear Regulatory Commission regulations and guidance, therefore, its impact on the current QA programs is expected to be minimal.

Modate on an SDMP Site - Pawlino. New Y.0r_h On December 22, 1992, Nuclear Regulatory Commission staff 'were provided a report on the underwater investigation of the " targets" that had previously been identified by magnetic and radar studies in-Nuclear Lake in Pawling, New York. Nuclear Lake is part of the Chevron Site Decommissioning Management Plan (SDMP) site. The public has been concerned that low-level wastes were previously disposed of in the lake. The report discusses the underwater investigation of the " targets" that took place and the results of the investigation.

The " targets" were determined to be rocks, tree stumps, a sunken boat, and a sunken Jeep.

No containers of waste were discovered and there _was no indication that radioactive material had been disposed of in the lake.

" Identification and Assessment of_BWR In-Vessel Seltra 3,gjdert Mitiaation Strateales" A comprehensive review and assessment of BWR in-vessel accident management strategies was recently published as NUREG/CR-5869, " Identification and Assessment of BWR In-Vessel Severe Accident Mitigation Strategies."

In the unlikely event of severe accidents involving " multiple" system or equipment failures, two accident management strategies were identified and assessed:

(1) containment flooding to cover the vessel lower head with water (vessel insulation is not air or water tight), and (2) boration of injection water to prevent recriticality in a damaged core in which control rods may have melted.

For existing BWRs, containment flooding can dalay vessel lower head failure by several hours if the flooding is executed witt :n about 2.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> after core damage initiation and if vessel insulation has sufficient leakage paths to allow water to contact the external vessel wall.

However, it is not realistic to consider containment flooding to maintain vessel integrity of existing BWRs.

To prevent recriticality during reflood of a damaged core in which control rod melting has occurred, adequate boration of the injection water is required. A practical means to provide boration of the injection water is identified.

items of Interest Week Ending January 1, 1993 Telephone Switchina System On December 29, 1992, a contract was awarded to Government Telecommunications incorporated (GTI) for the purchase, installation, and maintenance of four Telephone Switching Systems. This five-year contract will begin January 1,1993.

The systems will be at the NRC Operations Center located in Bethesda, Maryland, Region I, Region 111, and Region V.

Outdoor Telechones for MNBB and NLSB Weather-proof and vandal-resistant outdoor telephones' were installed at the Maryland National Bank Building (MNBB) and the Nicholson Lane South Building (NLSB).

The telephones are to be used to gain access to the buildings (summon an NRC guard) after hours if the card reader fails or employees forget their badges.

New NRC form The Office of Information Resources Management designed NRC Form 750, " Applicant Background Survey," for the Office of Personnel (0P) to replace the Office of Personnel Management Form 1386. The form will be used by OP in the recruitment process. All new employees will' submit the form with their applications.

New CD-ROM Product Scitech Reference Plui is-now available in CD-ROM format at the Nicholson Lane Branch Library, it contains a comprehensive source of information.on all scientific books and journals.

It also includes ' biographies of prominent American and Canadian scientists and engineers, and information on highly technical companies and research and development facilities.

New Employee Orientation--Personal Computer Briefina Starting in January 1993, IRM staff will participate in the new employee orientation program.

Information Technology Services Section staff will brief new employees on the personal computer program at NRC.

Office of Enforcement Items of Interest Week Ending January 1, 1993 Sianificant Enforcement Actions:

1.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $8,000 was issued on December 28, 1992 to Western Technologies, Inc., Phoenix, Arizona.

The action was based on the willful failure to wear alarm rate dosimeters when performing radiography in NRC jurisdiction, which has been categorized as a Severity Level 11 violation. Mitigation for good corrective action was offset because the NRC identified the violation.

(EN 92-100) 2.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $100,000 was issued on December 28, 1992 to Gulf States Utilities (Riverbend Station). The action was based on 15 violations in which radiation surveys were not performed as required, radiation protection procedures were not followed as required, and contaminated material was disposed of by transfer to an unauthorized recipient.

Collectively, these violations are considered to represent a Severity Level III problem. Mitigation for good corrective action was offset by escalation for poor prior performance in the health physics area.

However, in order to convey the significance of this violation, the civil penalty has been escalated by 100 percent of the base civil penalty.

(EN 92-101) 3.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount.of $100,000 was issued on December 28, 1992 to the Duke Power Company (0conee).

The action was based on a violation for the

-licensee's failure to take corrective action for.a condition adverse to quality.

Specifically, during the performance of a periodic test on-the Low Pressure Service Water system the potential for degraded cooling flow through the 38 Low Pressure Injection Cooler was identified, but not followed up.

Based upon questions from the NRC resident inspector during the subsequent unit outage, the licensee performed additional testing on the system and identified a partially closed manual valve that was restricting flow. This violation has been classified at Severity Level Ill. The base civil penalty of $50,000 has been escalated 50 percent for NRC identification and 50 percent for past poor performance. (EN 92-102) 4.

A Notice of Violation and Proposed Imposition of Civ'il Penalty in the amount of $62,500 was issued on December 28, 1992 to the Northeast Nuclear Energy Company (Millstone 3). The action was based on two violations involving the operation and testing of the unit's auxiliary building filter system and supplementary leak collection system.

Specifically, it was determined that the systems have been operated since plant startup in. configurations that would not ensure that accident condition radiological releases were properly processed and it was also identified that periodic testing of the systems was inadequate to detect the problems. Together the two violations have been classified as a Severity Level Ill problem. The base civil penalty in JANUARY 1, 1993 ENCLOSURE K

this case has been escalated 25 percent to $62,500 based on 100 percent escalation for prior opportunities to identify the problem, 50 percent mitigation for licensee identification, and 25 percent mitigation for corrective actions.

Civil Penalties Paid 1.

The University of Missouri, Columbia, Missouri paid the civil penalty in the amount of $625. The actions were based on two incidents in which shipping packages containing radioactive material produced in the research reactor were labeled with an incorrect isotopic activity and sent to the wrong recipient.

Metals Evaluation and Testing, Inc., Oakland, California paid the civil penalty in the amount of $7,500. The action was based on the failure to perform a thorough survey of a radiography exposure device to assure that the source had been returned to its fully shielded position use.

Region I Items of Interest Week Ending January 1, 1993 1.

Spent Fuel Route Surveys Route surveys for the transportation of spent reactor fuel to the Department of Energy's Savannah River Plant in Aiken, South Carolina, were conducted on December 29-30, 1992.

The surveys were conducted in preparation for shipments from the Massachusetts Institute of Technology Nuclear Reactor Laboratory in Cambridge, Massachusetts and the Rhode Island Nuclear Science Center in Narragansett, Rhode Island.

2.

FitzPatriqh On December 29, 1992, the Regional Administrator signed a letter that indicated agreement with the New York Power Authority's conclusion that technical and performance issues at the FitzPatrick Nuclear Power Plant were satisfactorily resolved to support restart.

FitzPatrick has been shutdown since November 27, 1991. As of 9:00 a.m., the plant is in cold shutdown and the licensee is performing minor repairs to components in several systems before restart.

Startup is currently scheduled for December 31, 1992.

R. S. Scott and Associates An enforcement conference was conducted with R. S. Scott and Associates on December 29, 1992, to discuss the loss of control over a moisture density gauge.

The licensee is instituting corrective actions involving instruction and training of workers and increased audits of field work by the management.

Escalated enforcement is being considered.

Paramax Systems Corooration An onsite inspection on December 17, 1992, into a previously reported high 'ilm badge reading showed that the exposure was to the badge only and di, not represent an overexposure to any individual.

Seouoyah Fuels Corooration On December 23, 1992, a modified Confirmatory Action Letter was issued to Sequoyah fuels Corporation (SFC) allowing startup of-the depleted uranium tetrafluoride -(DUF4) facility which had remained in shutdown mode following an event that occurred on November 17, 1992, at the-Sequoyah facility.

On December 28, 1992, the licensee began heating DUF4 process systems to bring them to operating temperatures. However, technical-difficulties were encountered.with a computer terminal in the facility control room which is used to monitor process systems in the DUF4 facility.

The licensee expects to replace the computer terminal today and will continue with startup activities after acceptance testing for the terminal is completed.

Positive Marijuana Results On December 1, 1992, a contract painting foreman had tested positive for marijuana after eating one of four cupcakes that-he found in his office area inside the protected area.

The foreman had turned himself in for a voluntary drug screening test when he became ill after eating the cupcake.

On December 29, 1992, Arkansas Nuclear One reported that the cupcakes involved in the drug incident tested positive for marijuana.

The licensee is performing an investigation in an attempt to determine the origin of the cupcakes.
