Enclosed is Quality Assurance Program Approval for Radioactive Material Packages No. 0639, Revision No. 1 This approval satisfies the requirements of 10 CFR 5 71.12(b) for a quality assurance program approved by the Commission. Registration for use of NRC approved packagings should be made pursuant to 10 CFR 6 71.12(c)(3).
Please note the conditions in the approval.
The " Radioactive Material Log" and " Notice To Employees," attached to your QA Program submittal, are not required for a QA program approval. These documents were not reviewed.
