NOTICE OF NONCONFORMAt4CE
This refers to your response dated December 3, 1992, concerning the Notice of
Nonconformance identified in the U.S. Nuclear Regulatory Coamission Inspection
Report No. 710122/92-08, dated November 4, 1992.
The corrective actions which have been or will be taken regarding
tionconformance B are acceptable.
Concerning your denial of Nonconformance A, please note that Revision No. 3 of
your QA document was issued with different dates. Specifically, Page ii of
J. L. Shepherd & Associates (JLS&A) document QA-RM-001-A indicates that
Revision flo. 3 of this document was revised in entirety on 10/10/90. Revision
No. I was issued on 01/03/91, Revision No. 2 was issued on 02/15/91, Revision
No. 3 was issued on 03/05/92, and Revision No. 3 was issued again on 08/01/92.
In industry practice the revision level of a document is changed to a higher
number or letter whenever an alteration, may it be minor, is made to that
document on a date different than the previous revision. The current JLS&A
document control system is inadequate for establishing identification of the
most current version of a document by revision level alone.
Please assess your document control program to ensure that the issuance of
revisions to documentation is controlled in such a manner as to avoid multiple
releases of the same revision number on an altered document.
Unless you object to our stated resolution of Nonconformance A, we consider
all items of nonconformance in the subject inspection Report resolved.
