SUBJECT: REQUALIFICATION PETAKE EXAMINATION AND INSPECTION

REPORT 50-245/92-32 (OL)

This refers to the inspection conducted and requalification retake examinations administered

-

by Mr. D. Florek and others frora December 14 - 17, 1992, at your Millstone Unit 1

Nuclear Power 50ttion. The examination was conducted in accordance with NUREG-1021,

" Operator Licensing Examiner Standards," Rev. 6. The inspection reviewed licensee actions

on prior emergency operating procedure issues, independent audits of operator training -

progrcms, and corrective actions identiDed at the December 3,1992, management meeting in

the Region ottices.

On December 17, 1992, at the conclusion of the examination and inspection, findings related

to the preparation and administration of the requalification retake examinations and inspection

findings were discussed with Mr. Ilaynes and other members of your staff identified in the

enclosed report.

Four out of the five operators examined passed the retake examination. One operator failed

in the emergency classincation part of the simulator examination. One operator that was -

scheduled for the simulator retake examination was withdrawn by the facility on the morning

of the examination.

We also acknowledge your revised implementation date of July 1993 for Bowchart based

emergency operating procedures (EOP) as identified in your December 2,1992, letter.

Section 3.0 reviewed your implementation of prior EOP issues in your flowcharts.

Section 4.0 contains an unrese'ved item regarding how the technical specifications

requirements of paragraph 6.5.3.7 to audit training of the unit staff were being met. Based .

upon the information available to the inspector, it is not clear that audits of operator training

were performed in the period of 1986 to 1992.

The report also discusses the December 3,1992, management meeting at the Region ofGees

in which you presented the facility analysis of why the 1991 remediation effort failed to

, prevent the 1992 weaknesses and the corrective actions that the facility will take. Your letter

'

dated December 7,1992, which was submitted as required by item 6 of our conGrmatory

action letter CAL l-92-014, also provided short and long term corrective actions and the

i

status of previously committed long term corrective actions. Your corrective actions will be

assessed further in future inspections of your licensed activities,

in accordance with 10 CFit 2.790 of the Commission's regulations, a copy of this letter and

the enclosures will be placed in the NRC Public Document Room.
