" Failure to Follow Procedure to Prevent Dissimilar Metallic Materials From Coming in Direct Contact With One Another" - Severity Level V.

actions were taken at the time the discrepancy was identified The carbon steel platform that came in contact witn the reactor E
vessel cladding was removed and covered wfth "A" cloth, a low-halogen i
material which isvapproved for contact with stainless steel.

addition, the affected areas of the stainless steel cladding were O
thoroughly recleaned before resuming ' work.
The "A" cloth covering will remain on the personnel basket as long as there i~s a possibility B
of contact with stainless steel.

To prevent further violations, NISCO supervisory personnel were a
reindoctrinated in the contents of engineering specification ES-67 E
and were admonished to maintain adequate control of activities i
under their charge.
" Gang box" meetings were also held with craft

personnel to ensure they are aware of the seriousness of contamination of metallic materials resulting from contact with

dissimilar metals and steps that should be taken to prevent this contact.

All corrective actions have been completed and full compliance
