Dear Mr. O'Connor:
This refers to the special safety inspection conducted by Mr. Wayne Slawinski and Ms. Michelle Barry of this office on November 6, 1992, of activities authorized under the general license provisions of 10 CFR 31.5, and to the discussion of our findings with you and others of your staff at the conclusion of the site inspection on November 6, 1992 and in a telecon with you on December 9, 1992.
The inspection was an examination of activities conducted under your general license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license.
The inspection consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During this inspection, certain of your activities were found to be in violation of NRC requirements, as specified in the enclosed Notice.
A written response is required.
Because you did not commit to specific corrective actions, including measures to prevent recurrence, during this inspection, the criteria set forth in Appendix C, Section Vll.B.1 of the NRC Enforcement Policy,10 CFR Part 2, dated February 18, 1992, were not met.
Therefore, Severity Level V violations have been included in the enclosed Notice.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letters the enclosure, and your response to this letter will be placed in the NRC Public Document Room.
The response directed by this letter and the accompanying Notice are not subject to the clearance procedures of the Office of Management and. Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
