RECOGNITION OF RECIPROCITY This acknowledges receipt of your Report of Proposed ' Activities in Non-Agreement States (NRC Form 241) (dated December 4,1992) and receipt of your
$640 reciprocity recognition fee (check dated November 30,_1992) as required by 10 CFR 150.20(b)~and 10 CFR 170.31 (Category'16-Reciprocity).
Your Alabama State License. No.147 is recognized-as valid for use in.Non-Agreement' States for up to'.180 days in a calendar year. In calendar. year 1993, a new NRC Form 241 and reciprocity fee 1will be required. This-notification must be made at least 3 days in advance of beginning licensed activities. You-must notify us concerning any revisions to_the activities (i.e., client, location, dates), by letter, containing the same .information as reported in Sections 1-7 of NRC Form 241. The certification provided in Section 8 of the first calendar year submission.NRC Form:241 will continue to apply-to all the activities. There is no additional fee required beyond the first submittal of the calendar year.
You must also comply with our 10 CFR Part 34 requirements concerning-radiography equipment, alarming ratemeters, and radiographers and assistant radiographers; NRC rules may-differ from Alabama rules. You must-also obtain an NRC-approved Qual'Ity- Assurance Plan: for shipping packages. Information on applying for the Quality Assurance Plan is enclosed (Regulatory Guide 7.10).
You must also register with NRC as a secondary user of'a package for which NRC-has issued a specific license, certificate of compliance, or other approval under 10 CFR Part 71. -Copies of portions of Title 10 Code __of Federal Regulations are listed as Enclosures 1-8.
The NRC may inspect your- activities in the field. -If an -inspection'is
= performed, you will be billed. The-fee charged will be the same as that charged to a similar NRC licensee. We will notify the appropriate State concerning your activtties.
