With regard to Category 2 of your request, the NRC has no documents. The NRC does not subject companies to criminal actions; rather, the U.S. Department of Justice is responsible for prosecuting all federal criminal actions.
With regard to Category 3 no companies have been debarred from government contracts by the NRC since 1987. -
