SUBJECT:
NRC !NSPECTION REPORT NOS. 50-445/92-51; 50-446/92-51 (NOTICE Of
VIOLATION)
This refers to the inspection conducted by Mr. D. N. Graves, and the
inspectors identified in the attached report, of this office on October 25
through December 5, 1992.
The inspection included a review of activities
authorized for the Comanche Peak Steam Electric S ction, Units 1 and 2,
facility. At the conclusion of the inspection, the findings were discussed
with you and those members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
The violation is of concern because the failure to
perform the recommended testing of TERMI-POINT connections allowed deficient
conditions to remain uncorrected for approximately 2 years in the Unit I solid
state protection system.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence,
it is expected that your response
will adt ress the impact of these specific deficiencies on the operability- of
tae equipment involwed and discuss all other systems identified in-
Westingi ouse Technical Bulletin NSD-TB-89-06.
After reviewing your response
to this Notice, including your proposed ::orrective actions and the results of
future inspections, the NRC will determine whether future NRC enforcement
action s necessary to ensure compliance with NRC regulatory requirements.
