SUBJECT:

NRC INSPECTION REPORT NOS. 50-445/92-47; 50-446/92-47 (NOTICE OF

VIOLATION)

This refers to the inspection conducted by Mr. W. B. Jones, and the inspectors

identified in the attached report, of this office on October 11 through

November 25, 1992.

The inspection included a review of activities authorized

for the Comanche Peak Steam Electric Station, Units 1 and 2, facility. At the

conclusion of the inspection, the findings were discussed with you and those

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activitles in progress.

Based on the results of this inspection, certain licensed activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The first violation reflects a lack of awareness for

controlling material used on safety-related components.

The second violation

is of particular concern because rigging activities involving heavy loads

around the reactor vessel head could have caused significant damage to the

refueling cavity wall or reactor vessel head components.

Two unresolved items were identified during this inspection.

One involved the

configuration control process in place and the corrective actions taken in

response to licensee identified events. -The second involved radiological

worker practices relating to recent contamination events that resulted in

uptakes to contract employees. -These matters are discussed in Sections 2.3

and 2.4, respectively, of the enclosed inspection report.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether future NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements,

it was identified that the repair to the atmospheric relief valve remote

operator arm was in violation of your work control program.

This violation is

not being cited because the criteria in paragraph Vll.B.1 of Appendix C to

10 CFR Part 2 of the NRC's " Rules of Practice" were satisfied.

This violation

was an isolated occurrence and the licensee staff inok prompt and effective

actions to correct the problem,

in accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy

of this letter and its enclosures will be placed in the NRC Public Document

Room.

The response directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
