Specifically you requested that the District provide a systematic safety assessment program to assess unscheduled reactor trips (SER Section III.E).
Attached is the Districts procedure which requires a systematic safety assessment for post-trip review, AP-28, Post Trip Transient Report. The District has reviewed the guidelines in your SER and considers that this procedure provides a systematic safety assessment to assess unscheduled reactor trips. You should note that the procedure uses the term "most probable cause" rather than " root cause". The District uses the phrase
" root cause" for the followup or longterm assessment which is a deliberate and thorough investigation that is normally not completed until after startup. Also, the procedure allows the Shift Supervisor to delegate the authority to reconstuct the transient to a Senior Reactor Operator. But, the Shift Supervisor still is required to review and approve the trip investigation.
