Dear Dr. Murley:
IE BULLETIN 83-06 NONCONFORMING MATERIALS SUPPLIED BY TUBE-LINE CORPORATION NO. 1 AND 2 UNITS SALEM GENERATING STATION DOCKET NOS. 50-272 AND 50-311 In our response to Bulletin 83-06 dated August 29, 1984, we indicated that further Nondestructive Examination (NDE) was required for the Tube-Line fittings installed in the Unit No. 2 chilled water system.
A review of the original radiographs had indicated that the construction welds for these fittings were masked.
The fittings were believed to l
be acceptable in all other respects due to the extensive non-destructive and destructiva evaluation which had been performed previously.
i Additional NDE (i.e., MT and RT) was performed on February 26, 1985 to establish the integrity of the fittings in the vicinity of the construct. ion welds.
Upon review of the radiographs, potentially rejectable indications were noted in the construction welds themselves.
In order to eliminate all uncertainty surrounding the integrity of the installation,.all nine (9) Tube-Line fittings have been replaced with qualified carbon steel fittings installed under closely supervised welding procedures.
This work and subsequent hydrotesting was completed on March 5, 1985.
