Office of Nuclear Reactor Regulation Items of Interest Week Ending December ll, 1992 Millstone 2 Millstone 2 is in the 194th day of the outage for replacement of the steam generetors. The replacement project is approximately 86 days behind the original schedule with startup (reactor heatup) projected for approximately December 24, 1992. Both old steam generators have been successfully removed from the containment and both have been delivered to the burial site in South Carolina and are ready for burial. Both new steam generators are in place in the containment. An audit of the licensee's stress analysis relating to the springing of the cold leg piping to the steam generators is in process. The licensee has moved fuel back into the reactor vessel. The hydro testing of the new steam generators is in process. Preparations are underway to perform the containment integrated leak rate testing. The staff is reviewing the request for technical specification changes related to the MSLB reanalysis and the loss of norm 61 power event. These issues have involved several discussions with the licensee's staff resulting in additional submittals containing requested information. Both amendments rieed to be and are presently uheduled to be issued before startup, bl1Ldel Representatives from NRR, HMSS and Region III attended a December 8, 1992, Van Buren County Commission meeting in response to a request by the county commissioners to have NRC address issues related to dry cask spent fuel storage at Palisades. The meeting was hosted by Palisades and was held on site with several of the county commissioners touring the plant before the meeting. Fritz Sturz from NMSS provided an informative and comprehensive presentation on dry cask storage, and all NRC personnel answered questions asked by the county commissioners and members of the public. Several local intervenor groups attended the meeting and a small group protested at the t plant access road. The most active groups at Palisades

• Palisades Watch" and
• Don't Waste Michigan," hava received considerable press caverage in recent weeks, as did the meeting with the NRC.

The meeting provided an opportunity for the staff to commur.icato directly with the public and inform the county commissioners on the process by which the licensee's decision to use dry cask storage is implemented through NRC regulations. After the NRC representatives left, the county commissioners continued with their meeting. We understand that the meeting then became confrontatienal and the local police were called in as a precaution to provile an escort for the commissioners. The rulemaking, which certifies the Palisades storage cask, is in its final stages. All comments received during the public comment period have been resolved and a CRGR meeting on the final rule is scheduled for December 18, 1992. DECEMBER 11, 1992 ENCLOSURE A

I 2 1ro.ian Portland General Electric Company, the licensee for Trojan Nuclear Plant, informed the staff by telephone on December 4, 1992, that it will withdraw its request for an emergency technical specification change regarding the steam generator.nspection issue. The licensee decided to complete additional inspections of the steam generators using the correct size bobbin coil, and perform other additional inspections as necessary. This inspection is in addition to the current data review already in progress. The licensee plans to submit a new technical specification change request on or about December 22, 1992, and will request exigent circumstances rather than emergency circumstances. NRR and Region V personnel are closely monitoring the status of this issue. On December 1, 1992, a public meeting was held at the Trojan site between the licensee and the staffs of Region V and NRR to discuss the steam generator inspection issue. The licensee made a forni presentation which was followed by a question and answer session. The meeting was attended by representativet, of the Oregon Department of Energy and the Energy facility Siting Council, who were allowed to ask questions of the licensee. Additionally, the meeting was well attended by members of the general public, moat of whom expressed concern about the continued operation of Trojan with steam generator problems. Also, significant local broadcast and print media were present. After the meeting, several people were arrested for civil disobedience after attempted entry onto the Trojan site. On December 3, 1992, another arrest for civil disobedience occurred as well. Finally, the NRC has received two letters from local Oregon grou)s regarding Trojan operation with steam generator problems, in addition to tio letters from the Union of Concerned Scientists, in general, the letters request some type of meeting or hearing on steam generator matters. Dnjsn Chanae Inspection at Browns Ferry On December 4,1992, the Special Inspection Branch completed a design change-inspection at Browns ferry Nuclear Plant. Selccted design change packages issued for Units 2 and 3 during the last two years were reviewed. The team identified strengths in the control of the design change process, discipline interface coordination, and retrievability of documentation. The team identified deficiencies regarding the design of the hardened wetwell vent, the excess flow check valve setpoint for isolation of control air header to each unit, inadequate justification or documentation for data used in calculations, acceptability of routing water piping over electrical equipment, and the basis for approval of field changes. The hardened wetwell vent design for Unit 2 requires manual operation of three drain valves (located in the reactor building, the yard and inside the stack) to drain the vent line. The team did not find any aralysis demonstrating the acceptability of the dose to the operator due to such proposed manual operations. The licensee stated that their criteria for the design specified no fuel failures and was consistent with the criteria provided by the BWR { DECEMBER 11, 199E ENCLOSURE A

3 Owners Group and their interpretation of the intent of the NRC guidance. The team discussed this issue with SCSB and projects, and concluded that the dose to the operator should be evaluated since the emergency procedure guidelines retuired venting if containment pressure exceeded the limit irrespective of the offsite radioactivity release rate. A meeting (tentatively scheduled for arranged)by projects.between the staff and the licensee to discuss this issue is bein 12/16/92 Drua Testing.Jdvisory Board Meetina PSGB:DRSS staff attended the December 3,1992, meeting of the Drug Testing Advisory Board from the National Institute on Drug Abuse (NIDA). NIDA announced that its proposed revision to the HHS mandatory testing quidelines was still being processed by OMB. 00T announced that its alcohol testing regulations (some 2,000 > ages) would be published in the Federal Register in the very near future. 11e DOT regulations consist of 18 separate FR Notices, including one devoted to data collection. During the sessions closed to the public, several studies and their policy implications were discussed. One study addressed test sample dilution and the effect of dilution on the overall positive testing rate. The study's results imply that dilution of specimens is a factor to be considered in establishing policies in the testing process. Meetina With NUMARC and EPRI on MOV Performance Prediction On December 3,1992, the NRC staff held a public meeting with representatives of the Electric Power Research Institute (EPRI) and the Nuclear Management and Resources Council (NUMARC) to discuss the progress and status of the Motor-OperatedValve(MOV)EPRIdiscusseditsresponsetotheNR.Cstaff'squestions Performance Prediction Program being conducted by EPRI. During the meeting, on the program that had been previously sent to EPRI. The staff commented on EPRI's plans during the meeting and are preparing minutes of the meeting. For example, the staff continues to have questions about (1) the applicability of the EPRI methodology to the population of valves.in the industry and (2) the criteria that will be used to demonstrate that the EPRI methodology is a>plicable to specific valves installed in nuclear plants. EPRI stated t1at it would provide additional information in these and other areas to address the staff's concerns. In response to the staff's questions, EPRI also stated that its program would not include consideration of aging effects on MOV performance. The staff considers )otential aging effects to be an important issue that needs to be addressed )y the industry.- The staff will discuss the most ap3ropriate means to address the aging issue with NUMARC. The staff intends to 1old another meeting with EPRI on the MOV Performanco Prediction Program in the Spring of 1993. The installation of advanced computing capability in NRC took a significant step forward last week with the establishment of Internet access for the engineering workstation network. Close cooperation between IRM and DECEMBER 11, 1992 ENCLOSURE A

1 4 NRR/DSSA/SASG resulted in implementation of the Internet link, which provides direct access for selected staff members with staff at the national r laboratories and with other technical organizations throughout the world. This high speed networking access )ermits the transfer of data files and code files electronically, rather than )y magnetic tape and the mall system. The result is code and file updating in a matter of minutes instead of weeks. Additionally, Internet provides sophisticated electronic mail transfer, news, and other services to technical, research, and educational users throughout the world. 1hermo-Lao Testina for Comanche peak Unit 1 A Thermo-Lag 330-1 fire barrier fire endurance test was performed by Omega Point laboratories, San Antonio, Texas for Texas Utilities' Electric Company on December 3, 1992. Preliminary results indicate that the assembly did not satisfy agreed upon criteria for rated fire barriers. The test assembly consisted of 1%-inch, 3/4-inch, and 2-inch conduit specimens with lateral bends (LDDs) and radius bends. The fire barriers included a combination of vendor recommended assembly methods and licensee designed upgrades intended for potential application to Unit 1 installations. The 3/4-inch conduit was upgraded to a nominal thickness of 3/4 inch. The thicknesses of the 1%-inch and 2-inch conduits were not upgraded but left as installed (nominal 1/2 inch). In addition to recording the maximum and average temperatures fer the various samples, the following are some of the results noted by the licensee: burn through was noted on the 1%-inch and 2-inch conduit barrier material; Megger test was satisfactory; some minor jacket swelling was noted on the 14-inch conduit cable at two places and 2-inch cable conduit. The conductor insulation was undamaged. ligetino on AP600 Intearal Testing The staff met with Westinghouse on December 9 and 10, 1992, to discuss Westinghouse's integral systems test programs in support of AP600 design certification. The December 9 meeting covered the reduced-height, low-pressure tests at Oregon State University, and the December 10 meeting was devoted to the full-height, high-pressure tests in the SPES-2 facility at the SIET Laboratories in Placenza, Italy. A meeting is scheduled for December 14, 1992, to discuss overall status of the AP600 test program. DECEMBER 11, 1992 ENCLOSURE A

1 Office of Nuclear Material Safety and Safeguards i Items of interest Week Ending December 11, 1992 Duality Assurance inspection On December 1-3, 1992, the Transportation Branch staff conducted an inspection at Public Service of Colorado, Fort St. Vrain in Platteville, Colorado. The inspection team reviewed implementation of the quality assurance program in the procurement, maintenance and use of NRC-certified spent fuel packagings. Preliminary findings were discussed with company representatives at the exit meeting. Presentation to DOE on Quality Assurance Inspections On December 2,1992, a representative of the Transportation Branch gave a presentation on the NRC quality assurance inspection program to the Transportation and Packaging Safety Division, Department of Energy (DOE), and their contractors. The NRC presentation addressed the history, inspection methodology, findings, and staff training of the quality assurance inspection program for transportation. Task Force Meetino on Reportino Reauirements Two members of NMSS are representing the office on the Task Force on Reporting Requirements. The purpose of the task force is to review agency reporting recuirements for power reactor facilities to determine which ones are recundant or can se eliminated or revised. NMSS representatives were assigned the task of reviewing 10 CFR 70.9, " Completeness and Accuracy of Information"and73.71(c)'ReportingofSafeguardsEvents." MC&A at Portsmouth Diffusion Plant On December 9,1992, members of the Division of Safeguards and Transportation met with a representative of DOE to discuss MC&A at the Portsmouth plant. The information gained will be used in drafting the standards *, hat will govern MC&A at the plant after its operation is taken over by the U.S. Enrichment Corporation. Safeauards Bilateral Meetinas During the period November 30, 1992 through December 11, 1992, a member of the International Safeguards Branch participated as a member of an interagency team in safeguards bilateral' meetings in the Federal Republic of Germany, Luxembourg, France and the United Kingdom. Topics of discussion with respective government representatives and the EURATOM inspectorate included the status of the implementation of International Atomic Energy Agency (IAEA) safeguards, proposed new IAEA safeguards initiatives, safeguards for nuclear wastes, cooperation in safeguards research and development and IAEA annual Safeguards Implementation Reports. l!. 2 Briefina on General Atomics Test Reactors On December 9,1992, Mr. Anthony Vaca, Test Reactor-Isotope-General Atomics (TR!r.A) Group, General Atomics, San Diego, California briefed interagency representatives on the TRIGA characteristics and capabilities and provided comments on future program activities. Staff from the Departments of State, Energy, and Commerce, the Arms Control and Disarmament Agency and the Nuclear Regulatory Commission attended this briefing. Audit of Office of Civilian Radioactive Weste Mannaement (OCRWM) During November 30 through December 10, 1992, the quality assurance (QA) staff of the Division of High-ievel Waste Management (HLWM) observed a limited scope internal QA audit of OCRWM. ihis audit, HQ-93-01, evaluated the OCRWM QA program to determine whether it meets the OCRWM requirements and commitments. Seven QA programmatic element tr se ' @ d by the audit team. The HLWM observers determined ' m.y c p ~ effective and agreed with the audit team that implementation Jo : a'. O program was generally adequate v in all of the QA programmatic eleuc audited. As a result of the audit, eight preliminary Corrective Action Recuetts (CARS) were issued by the audit team. The adverse conditions identifin in the CARS are not significant in terms of the overall OCRWH QA program. Update on SDMP Site - Pawlina. NY On December 1, 1992, Chevron corporation submitted a Work)lan and a Health and Safety Plan for the remediation of contaminated soil at tie Pawling site. Chevron seeks expedited NRC approval to excavate previously identified contaminated soil, which would allow for the disposal of the soil at a licensed disaosal facility prior to December 31, 1992. A work 31an for completing tie Pawling site decommissioning, i.e., remediating auildings and remaining soil and performing a termination survey, will be submitted by Chevron in early 1993. The underwater visual search for the " targets" identified in Nuclear Lake during previous ground penetrating radar and magnetometry surveys is complete. This search was conducted by a contractor for the National Park Service. The verbal report from the contractor indicates that approximately 75% of the targets were positively identified. No metal objects, i.e., 55-gallon drums, were found. The majority of the targets were large stones. Since the lake was reported to contain little sediment there is low probability that a target was visually missed due to it being covered by sediment. The staff will evaluate the final report to determine if any additional investigation of Nuclear Lake is required. Boyd County Nebraska Votes Aaainst LlW Disposal Facility On December 9, 1992, the citizens of Boyd County, Nebraska voted in a straw poll to determine whether there was community consent for the new LLW disposal facility being developed there. Ninety-three percent of the voters opposed such a facility, and Governor Nelson has stated that he will now seek a court DECEMBER 11, 1992 ENCLOSURE B

3 ruling on whether the vote allows Nebraska to stop the siting process. It is unclear how the court might rule, since the definition of " community" is unclear. In this vote, the entire county was polled. Previously Butte, Nebraska the town closest to the site, had passed two resolutions in support of the facility. Reportedly, local residents in Butto boycotted the poll protesting that the >oll was nonbinding on the siting / licensing process. State law requires tlat every effort be made to obtain community consent, consistent with protection of public health and safety and the environment. The Central Interstate low-Level Radioactive Waste Commission believes that the process followed in the earlier selection of the low-level waste site complied with Nebraska State law. Meetina with Jananese Deleaation On November 30, 1992, staff met with a Japanese delegation that was visiting the United States (US) to examine US regulations and views on the disposal of depleted uranium (DU) tails. Edlow International Company, representing the Japanese delegation, contacted the NRC to request the meeting. The Japanese agenda centered on depleted uranium storage measures, including chemical form; storage containers; type of storage facility; storage schedule for fuel cycle facilities; disposal of " recovered hydrofluoric acid;" and NRC regulatory framework. The Japanese have a fast-breeder reactor program, and consider the DV a resource, not a waste. The Japanese delegation had made arrangements to meet with other government agencies as well as with private industry to discuss the same topic. DECEMBER 11, 1992 ENCLOSURE B

The Ad Hoc Working Group was established under FCCSET to review and assess current federal approaches to dealing with scientific and technical information and recommend necessary modifications. The first meeting of the group will be on December 21, 1992. An agreement between EG&G Idaho, Inc. and TRACTABEL of Belgium has been completed. TRACTABEL has agreed to contribute an agreed upon amount for the 5 year term of the agreement and to exchange information and share their research results. Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending December 11, 1992 Hiaanostic Evaluation and Incident Investiaation Branch (DEllB) The incident Investigation Team (IIT) continued its investigation efforts involving the potential overexposure of a nursing home patient as well as other individuals who may have come in close proximity to the unshielded source. On December 7, 1992 Oncology Services Corporation reported a second event at a different facility involving another high dose rate (HDR) after loading brachytherapy unit Some of the circumstances surrounding this failure app (Omnitron Model 2000). ear to be similar to those of the first failure. As a result, the llT leader and several team members, who are investigating the Indiana, PA incident were sent to the licensee's Pittsburgh facility to investigate this second event. An addendum to the Ili Charter was issued to include an investigation of the probable cause(s) for the failure of the second HDR unit. With respect to the second event, the licensee informed the NRC that a 3.5 curie, iridium-192 sealed source broke off of its drive cable while treating an B1 year old female patient for bronchial cancer. The catheter was cut, and the patient was removed from the treatment room. The source was recovered and placed in a shielded container. According to the licensee, the patient and the licensee's senior physicist received a negligible radiation dose as a result of the malfunction. The senior physicist agreed to preserve the equipment involved in an "as-found" state until the licensee and the llT had an opportunity to evaluate the event. As a result of these events, an NRC Bulletin has been issued to all brachytherapy licensees authorized to use the Omnitron Model 2000. Region I has sent inspectors to the licensee's Exton and Lehighton, Pennsylvania facilities to review suspected or indicated programmatic weaknesses uncovered by the 11T, including training, procedures, and any actions taken by the licensee at these sites as a result of these events. The llT will continue its activities during the next week, returning to the AE0D offices on Monday, December 14, 1992. On December 9, the DEllB Branch Chief participated with NRR and Region til in, a quarterly review of the Commonwealth Edison (Ceco) older plants. This was followed by his particiption in a Region Ill/ Ceco management meeting on December 10, during which the results of the quarterly review were discussed. A senior engineer from DEllB )articipated with Region IV in an OSTI at the South Texas Nuclear Plant. 111s individual inspected the engineering and technical support functional areas. The onsite portion of the OSTI was completed this week and report preparation has begun. Reactor Ooerations Analysis Branch Case Study AE0D/C92-01, ' Human Performance in Operating Events - 1992." which describes the results of a program begun in 1990 to conduct onsite, in-depth DECEMBER 11, 1992 ENCLOSUR't 0

studies of human performance that affected reactor safety during selected power reactor events. The purpose of the program is to identify the factors that have contributed to good operator performance during events as well as the factors that hindered performance and to feed this information back to industry. Under the human performance study program, six site visits were conducted in 1990, seven in 1991, and three in 1992. Each site visit was conducted by a multidisciplinary team, lead by an AE00 staff member, with additional NRC headquarters, regional, and Idaho National Engineering Laboratory personnel. The events studies include a wide variety of accident scenarios, including: stuck open safety-relief valve, reactor trip with safety injection, reactor scram due to positive reactivity insertion, reactor scram due to control rod withdrawal, pressurizer spray valve failure, partial loss of instrument air in containment, turbine building pipe rupture, loss of shutdown cooling, excess steam demand event, main steam isolation, loss of electrohydraulic fluid, and reactor water cleanup isolation defeated. The case study provides information on control room staffing and organization, the ' dual role" shift technical advisor, use of shift resources during emergencies, operator control of engineered safety features, simulator training, crew teamwork during stressful situations, task awareness, use of procedures, the human-machine interface, and licensee followup on events. The information could be useful to licensees in efforts to upgrade existing programs to improve safety. Preliminary Notifications: a. Rulemaking Activities - Disposal of Waste 011 by Incineration A final rule entitled, ' Disposal of Waste 011 by Incineration" was published in the Federal Register on December 7, 1992 (57 FR 57649). The final rule allows licensees to-incinerate contaminated waste oils generated at nuclear power plants-at the site without obtaining an amendment to existing operating licensees. The final rule also completes action on a petition for rulemaking submitted by the Edison Electric Institute and the Utility. Nuclear Waste-Management Group (PRM-20-15). The final rule becomes effective on January 6, 1993. Revised Standards for Protection Acainst Radiation: Minor Amendmentt A fina11 rule entitled, " Revised Standards for Protection Against Radiation; Minor Amendments" was published in the Federal Register on December 8,1992 (57 FR 57877). The final rule makes a number of corrective and conforming' amendments to the revised standards for protection against radiation (10 CFR 20.1001-20.2401). Office of Information Resources Management Items of Interest Week Ending December 11, 1992 foreian Document Biblioorachy On December 7, 1992, the November 1992 edition of the foreian Documents Biblicarachy, compiled and edited by Library staff, was distributed to selected NRC staff. The NRC Library contains an extensive collection of foreign reports received through international agreements with the Office of International Programs and the Office of Nuclear Regulatory Research. When these reports are received in the Library, they are entered into an online database which indexes them by country of publication, report number, title, publisher, and publication date. The Foreton Documents Biblicaraphy is a list of recent additions to this collection and is produced from the online database. Files Maintenance. Intearated Events in EXSIS The first phase of the Integrated Events enhancements to the Executive Shared Information System (EXSIS) have been completed and migrated to the production system. Horning Reports and Preliminary Notifications (in addition to Event Notifications) are now accessible in EXSIS, and reports are no longer limited to those related to Power Reactors. Though there are several things that need to be optimized (such as the time it takes to combine related reports) and some changes requested by customers that need to be incorporated, it was felt worthwhile to make the new information available at this time. A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $75,000 was issued December 8,1992 to Wisconsin Electric Power Company (Point Beach, Unit 2), The action was based on the inoperability of one train of the safety injection system piping in the recirculation mode due to the presence of foreign material in the piping. The violation continued for about a year and was due to inadequate foreign material-exclusion controls during a system-modification performed during the previous refueling outage. (EN92-093) 2. A Notice of Violation and Proposed Imposition of Civil Penalty 'in the amount of $25,000 was issued on December 8,1992, to the Philadelphia Electric Company. This action is based on a violation of the employee protection requirements of 10 CFR_50.7. The violation is based on a Department of Labor-Administrative Law Judge's-Recommended-Decision and Order finding that Protection Technology, Inc. (PTI), a contractor for the Philadelphia Electric - Company at the Limerick Generating Station, discriminated against an employee because he engaged in protected-activities. (EN92-094) 3. Notices of Violation and Proposed imposition of Civil Penalties in the amount of $137,500 was issued on December 9, 1992 to New York Power Authority.- The first civil penalty, in the amount of $100,000, was based on the failure of-the licensee to provide complete and accurate information to the NRC during an enforcement conference conducted on April 10, 1992,-to discuss a number of violations related to operability of the boric acid heat trace system, adequacy of alarm response procedures and the commercial dedication process for replacement fuses. The base civil penalty for this Severity Level III-violation was escalated 100%; 50%.for NRC-identification,-and 50% for inadequate corrective actions in that the licensee was still experiencing ~ internal communications problems regarding the: events two months after being put on notice of the violation. The second civil penalty, in the amount of $37,500, was based on the failure to obtain adequate medical assurance of an individual's fitness for duty before returning him to licensed duties and the failure to establish a follow-up program for that individual and another individual who had tested positive-for drugs. The base civil penalty for this Severity Level III problem was mitigated 50% for corrective action but was escalated -25% because the licensee-had prior opportunity to identify one of the violations. Office of State Programs Items of Interest Week Ending December 11, 1992 i Radiation Protection Enaineerina Course State Programs sponsored a " Radiation Protection Engineering" training course on December 7-11, 1992 at the Oak Ridge Institute for Science and Education facilities in Oak Ridge, Tennessee. The class consisted of 20 students from State radiation control programs. The course covered various aspects of radiation protection engineering including air sampling and ventilation, environmental dose assessment, accelerator health physics, particle sizing and x-ray shielding. NCSL State-federal Assembly Mindy Landau, State Programs, attended the National Conference of State Legislatures (NCSL) State-Federal Assembly on December 10, 1992 in Washington, DC. The NCSL Energy Committee and Environment Committee held a joint session on the Energy Policy Act of 1992 and on energy and environmental issues under the new administration. Charles Imbrecht, Chairman of the California Energy Comission and State Liaison Officer, gave the luncheon speech on the California Energy Plan. Availability of Document Maria Lopez-Otin, Federal Liaison, has received from the Carnegie Commission on Science, Technology, and Government its report " Enabling The Future: Linking Science and Technology to Societal Goals." Office of International Programs Items of interest Week Ending December 11, 1992 International Visitors On Thursday, Chairman SONG Jian, State Science and Technology Commission (SS10) of the People's Republic of China, visited the Commission. He was accompanied by Vice Chairman HUI Wong Zheng and officials of the Chinese Enibas sy. Chairman Song met with the Chairman and Commissioners Rogers, Remick, and Curtiss. In the meetings he expressed aapreciation for the valuable safety assistance the NRC has provided to tieir developing nuclear sower program and expressed the interest that a renewal of the Safety Protocol se signed soon. He made the point that with one power reactor now operating and two more units coming online in the next several years, safety assistance with the NRC in the area of safe plant operation is very important. Connecticut On December 8, 1992, Region I conducted an enforcement conference with Greenwich Hospital Association, Greenwich, CT. The conference was held to discuss the circumstances surrounding a therapeutic misadministration i to a patient that occurred from October 8, 1992 to November 9, 1992. ~ Violations c' licensee's quality management program resulted in the administrat* a radiation dose of 240 rads per treatment for 11 days instead of a escribed dose of 120 rads per treatment. The total dose received by toe patient was approximately 7440 rads instead of the l expected 4800 rads. Carolina P.gwer and Licht Comnany - Brunswick On December 7, an enforcement conference was held to discuss NRC findings from an inspection of an unplanned release of Americium-241. The release occurred when a source holder containing 5.4 Ci was inadvertently cut on September 22, 1992. Causes of the inadvertent cutting of a source included the failure to adequately evaluate the extent of possible radioactive hazards present prior to cutting the source holder and the inadequate material control of the 5.4 Ci Americium source including its location. The licensee addressed NRC concerns during the Enforcement Conference and did not take exception with the NRC findings during their presentation. On December 10, the Regional Administrator; the Director, Office Nuclear Reactor Regulation; and other members of the Region 11 and NRR staffs, conducted a public meeting with representatives of the Carolina Power and Light Company to discuss the

Prairie Island Nuclear Plant On December 9,1992, approximately 25 members of the Minnesota state legislature visited the Prairie Island Nuclear Station. The Minnesota Public Utilities Commission (MPUC) recently issued a certificate of need for the installation of 17 spent fuel storage casks at the Prairie Island site. The licensee expects that various groups opposed to the dry cask storage will attempt to introduce legislation in January 1993 to overturn the MPUC decision. Representatives from opposing groups, specifically, the Prairie Island Sioux Indian Community, the Prairie Island Coalition Against Huclear Stora marketing and public relations firm), ge, and Direct Expressions (a accompanied the legislators. A questions and answer session was scheduled to occur following the group's morning plant tour, and the Region 111 projects section chief and senior resident inspector were available to attend. Jalve Inspection - ODen Public Exit Meeting An inspection exit meeting, open to public observation, was held on December 7, 1992, at the Comanche Peak Steam Electric Station. At the meeting, the results of NRC's review of licensee actions taken to resolve issues related to Borg-Warner International Products pressure seal check valves were presented. Attendance at the meeting included members of the Citizens Association for Sound Energy (CASE) and the Citizens for fair Utility Regulation (CFUR), 2.
