in a pipe weld between the primary containrent penetration and the Entme,r e is ola tion Check Valve.
The cause of f a i l u e-o is not known at the present time; an investigation is in progress.
Additional inforntion including the cause of failure and corrective I
action will be included in a 14 day follow-up report.
