-On-October 19 the plant entered a 30 day administrative limiting condition for operation (LCO)'for maintenance on the out of service "A" Control Building Chiller.
The LCO was required by a plant special order and operating instructions, but not by.
the technical specifications. The "B" Control Building Chiller was in service to.
supply chilled water for the cooling coils of the associated air-conditioning unit.
The. chilled water system and air conditioning units maintain acceptable temperature
-for control room plant personnel equipment.
On October'22 at 12:56-hours,cwith the' plant at 99.9% -hermal power and the "A"
chiller still out of service, the "B"Echillwr tripped. Attempts to re-start the
-chiller were successful, but it' tripped again, after a short period of time, for unknown reascns. At'13:21 the "B" chiller was declared inoperable since operation could not be maintained, and a 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> administrative LCO was entered. As part of trouble shooting activities, maintenance M rsonnel discovered air in the chllled water piping which they vented. The "B" chiller was restarted at 15:10. At 16.10 the "R" chlller was declared operable and the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> LCO was exited.
The 30 day LCO for the "A" chiller remained in effect.- LER 92-016-(pending) aOn October 26 with the reactor operating at 100% a quality assurance = surveillance identified four missed hourly firewatches, two in the Realdual Heat Removal Valve Room on October 10, and two in the High-Pressure Coolant Injection Room on October 7 and 10.
The personnel who performed the firewatches failed.to prop rly use the firewatch checklist. All personnel who perform firewatches have since been instructed on the use of.and adherence to the firewatch checklist, All firewatch
. personnel will have additional -formal training on f he proper firewatch methods and-checklist usage.. Fire detection and suppression egipment in the affected' areas was operable throughout the event. The event had no offect on the safe continued operation of the facility. LER 92-017 (pending)
