4 An inspection of the Reactor Buildir.g revealed the following leakage paths:

(1) 'Ibe inner railroad airlock door was not closed properly and the packing in the railroad wheel flange croove was not in place.

(2) An exhaust duct in the trunnion room ward-awing air from the turbine building through a 1*SIV open for traintenance.

(3) The Trunnien Roor door was open to allow ventilation for the work being done on the MS1'V without the Trunnion Room exhaust duct being sealed.

Significance of the event ray be considered to be r.1".1 al in that while the SB3TS did not draw tha requ'"ed vacuu.,

its perforance was sufficient to :nitigate the consequences -

of a refueling acc$ dent.
