SUBJECT: NOTICE OF VIOLATION AND NRC INSPECTION REPORT NO. 50-461/92020(DRP)

Dear Mr. Perry:

This refers to the inspection conducted by Messrs. P G. Brochman,

F. L. Brush, and M. J. Miller of this office on October 27 through December 7,

1992. The inspection included a review of authorized activities for your

Clinton Power Station facility. At the conclusion of the inspection, the

findings were discussed with those members of your staff identified in the

enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of a selective examination of procedures

and representative records, observations of' activities in progress, and

interviews with personnel.

Based on the' results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation is of concern because it indicated a

weakness in the control of software in safety-related microprocessors as well

as a lack of understanding of what constituted maintenance. In your response

you are requested to specifically address whether your newly issued " software

QA" program adequately controls activities on the type of safety-related

microprocessor involved in this event.

You are required to respond to this letter and should follow the instructions-

specified in the enclosed Notice when preparing your response. In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements. In addition,

we request you provide a written evaluation within 60 days concerning_

potential radiological releases following an offgas charcoal bed fire, as-

discussed in.the unresolved item, identified in paragraph 2.b.
