Statement of Violation The Order for Modification of License, dated July 20, 1979, as amended by the Order dated February 11, 1980,. states in part:
... Pending further amendment of the Facility Operating License, the licensee shall maintain the facility in accordance with the requirements set forth in..." (Proposed Technical Specification, Appendix A to License No. DPR-73). The proposed Technical Specification 6.8.1.a states in part: " Written procedures shall be..._ implemented covering... the applicable procedures recommended in Appendix A ~of Regulatory Guide 1.33, Revision 2, February 1978...".
Specific activities referenced in Regulatory Guide 1.33 include equipment control.
Plant Maintenance Department Standing Order Memorandum No. 52, Air H'andling System Integrity, dated October 16, 1984, instructs that doors which may have been opened should have electric cords removed and the door closed when the area is unattended.
Contrary to the above, electrical testing leads were run through the door to the Auxiliary Building exhaust fan housing AH-E-8C/D which was held partially open with a wooden block.~ The test leads had been in place, the door opened, and unattended between March 20 - 25, 1985.
This is a Severity Level IV Violation (Supplement 1)
GPU Nuclear Response GPU Nuclear's investigation of the event determined that it was caused by a misunderstanding on the part of the job foreman. The job foreman was aware of the existence of Maintenance Standing Order Memorandum (M-SOM)
No. 52, however, he was not completely familiar with its contents. He believed that M-SOM No. 52 required only the minimization of the amount of time that the doors in the ventilation system cabinet could be left open. The M-SOM actually required that the doors in the ventilation system cabinet be closed "when the area is unattended...". As a result of this event, the following actions have been taken; 1.
The job foreman responsible for the event and his supervisor have been counseled on this matter.

The M-SOM No. 52 (attached) was discussed at a March 29, 1985, meeting involving all TMI-2 Maintenance Lead Foremen and Maintenance Managers. The intent of the M-SOM was discussed along with the specifics concerning this event.

The Inspection Report 85-08 was reviewed by all TMI-2 Maintenance Lead Foremen and Maintenance Managers.

GPU Nuclear believes' that.the actions detailed above are sufficient to preclude a recurrence of this event. Thus, GPU Nuclear considers that all necessary corrective actions have been completed and that full compliance has been achieved.

Joe October 16, 1984 Subject Air Handling System Integrity M-SOM # 52 Locahon TMI Nuclear Station To Distribution 4220-84-0421 Effective immediately, please minimize the duration that openings exist in the Auxilliary, Fuel Handling, and Reactor Building Ventilation Systems caused by opened doors, removed duct sections, removed instruments, removed capped connections into the duct, etc.
This memo is being issued in order to preclude inadvertant releases into the Auxilliary or Fuel Handling Building caused by a change in the ventilation status which could occur when the system is open for maintenance.
For example, doors in the past which may have been kept open for hoses or electric cords, should have these items removed when the area is unattended and the door closed.
Because a change in the ventilation status of a system does occur for many different reasons, to be conservative, we are taking this s.
precaution to prevent a possible contamination of the building or an unfiltered but monitored release to the environment. Planners, foremen, and workers must take this into consideration when working a ventilation job on the above mentioned systems.
